中文摘要 | 面對高齡社會,老年醫學整合性專業照護扮演相當重要的樞紐,然而在這持續性照護的過程中,緩和照護絕對是不可或缺的重要拼圖。在此提出一位高齡罹患重度失智與腎衰竭病人緩和照護之成功案例。醫療團隊藉由緩和醫療家庭會議與醫病共享決策方式,提供「老年醫學緩和安寧療護」無縫接軌之持續性照護,協助高齡病人及家屬得到身、心、靈之平靜,圓滿完成生命的最後篇章。我們於文中討論此類長者是否透析與使用鼻胃管之醫病共享決策依據及高齡長者緩和醫療之介入時機。 |
英文摘要 | Facing the aged society in Taiwan, geriatric collaborative care plays an increasingly important role, and palliative care is an essential link in the process of maintaining continuity of care. We provide the successful experience of caring an elderly patient with severe dementia and renal failure in the palliative ward. Our team held family meeting and applied the shared-decision making (SDM) concept to assist the patient and family to make the proper decision meeting the care goal of the patient's choice. The geriatric-palliative care team offered comforting holistic care in a seamless way from hospital to home to assist the elderly patient to retain good quality of life and bio-psycho-social-spiritual peace to the end. Evidence-based SDM tables are provided for facilitating discussions concerning hemodialysis for renal failure and nasogastric tube insertion in elderly patients with severe dementia and timing for initiating palliative care intervention. |
