中文摘要 | 孕婦產前B群鏈球菌(Group B Streptococcus;GBS;Streptococcus agalactiae;乙型鏈球菌)的檢測需使用運送、增菌以及分離培養基,常用的增菌培養液主要包括LIM broth及carrot broth,而分離平板培養基則包括BAP、CNA、CHROMagar StrepB (CHROMagar公司,法國)、GBS Detect^(TM)(Hardy Diagnostics)、β/γ GBS detection agar、GBS carrot agar與CMP^(TM) GBS Detection II(啟新生物科技有限公司,新北市,台灣)。在本研究中,進行了兩個階段的實驗來評估兩種檢測組合的效能,第一階段為在16件臨床檢體中比較Lim broth-BAP/CHROMagar StrepB與Lim broth-CMP^(TM) GBS Detection II檢測GBS的效能,第二階段則為於134件臨床檢體中比較Lim broth-BAP/CHROMagar StrepB與carrot broth-CMP^(TM) GBS Detection II檢測GBS的效能。於第一階段的實驗中,Lim broth-BAP/CHROMagar StrepB檢測出29/164件(17.7%)陽性檢體,其中Lim broth-CMP^(TM) GBS Detection II對其中2件檢體呈現陰性反應,而Lim broth-CMP^(TM) GBS Detection II則檢測出28/164件(17.1%)陽性檢體,其中Lim broth-BAP/CHROMagarStrepB對其中1件檢體呈現陰性反應。於第二階段的實驗中,Lim broth-BAP/CHROMagar StrepB檢測出26/134件(19.4%)陽性檢體,其中carrot broth-CMP^(TM) GBS Detection II對其中1件檢體呈現陰性反應,而carrot broth-CMP^(TM) GBS Detection II則檢測出36/134件(29.6%)陽性檢體,其中Lim broth-BAP/CHROMagar StrepB對其中11件檢體呈現陰性反應。盡管結果指出Lim broth-BAP/CHROMagar StrepB的檢出率為17.1%(28/164),而Lim broth-CMP^(TM) GBS Detection II為17.7%(29/164),但CHROMagar StrepB具有大約30%的偽陽性,需要檢查BAP生長菌落形態及溶血才能正確判讀結果。此外BAP/CHROMagar StrepB上懷疑為GBS的陽性標本中有6.36%(19/298)第二天確認為陰性,而在CMP^(TM) GBS Detection II懷疑為GBS的陽性標本中僅有2.01%(6/298)的檢體第二天確認為陰性。由於β-GBS在carrot broth及CMP^(TM) GBS Detection II的顯色具有診斷效益,因此,此組合的特性將可讓檢驗室節省人力與物力,且可提早發出報告。根據上述的發現,吾等認為carrot broth-CMP^(TM) GBS Detection II比Lim broth-BAP/CHROMagar StrepB更適合篩檢孕婦產前檢體中的GBS。 |
英文摘要 | Detection and identification of Group B Streptococcus (GBS; Streptococcus agalactiae) in specimens from pregnant women require transport, enrichment, and isolation media. Commonly used media for enrichment include Lim broth and carrot broth, and those for isolation include BAP, CNA, CHROMagar StrepB (CHROMagar, France), GBS Detect^(TM) (Hardy Diagnostics, USA), β/γ GBS detection agar, GBS carrot agar, and CMP^(TM) GBS Detection II (Creatives Co., New Taipei City, Taiwan). In this study, two separate experiments were performed with two different sets of specimens. The first experiment compared the efficacy of GBS detection between the combination of Lim broth-BAP/CHROMagar StrepB and the combination of Lim broth- CMP^(TM) GBS Detection II in 164 specimens. The second experiment compared the efficacy of GBS detection between the combination of Lim broth-BAP/CHROMagar StrepB and the combination of carrot broth-CMP^(TM) GBS Detection II in 134 specimens. In the first experiment, 29 of 164 (17.7%) specimens were positive by the Lim broth-BAP/CHROMagar StrepB combination; among them, 2 specimen was negative by the Lim broth-CMP^(TM) GBS Detection II combination. Twenty-eight of 164 (17.1%) specimens were positive by the Lim broth-CMP^(TM) GBS Detection II combination; of which 1 specimens were negative by the Lim broth-BAP/CHROMagar StrepB combination. In the second experiment, 26 of 134 (19.4%) specimens were positive by the Lim broth-BAP/ CHROMagar StrepB combination; among them, 1 specimens were negative by the carrot broth- CMP^(TM) GBS Detection II combination. Thirtysix of 134 (26.9%) specimens were positive by the carrot broth/CMP^(TM) GBS Detection II combination; of which 11 specimen was negative by the Lim broth/BAP/CHROMagar StrepB combination. Although the positive rate of the Lim broth-BAP/CHROMagar StrepB combination (17.7%) was similar to that of the Lim broth- CMP^(TM) GBS Detection II combination (17.1%), it had a false-positive rate of approximately 30% and required examination of colony morphology and hemolysis pattern on BAP. In addition, 6.36% (19/298) of the specimens suspected to be GBS positive on BAP/CHROMagar StrepB on the first day were confirmed to be negative on the second day. In contrast, only 2.01% (6/298) of the specimens suspected to be GBS positive on CMP^(TM) GBS Detection II on the first day were confirmed to be negative on the second day. As -GBS produced orange-red color on carrot broth/CMP^(TM) GBS Detection II, the turnaround time of this combination was shorter, and the costs for labor and supplies were lower. Based on results of this study, we conclude that the combination of carrot broth- CMP^(TM) GBS Detection II combination is more appropriate than all the other combinations tested for GBS screening in prenatal specimens from pregnant women. |
