| 中文摘要 | 近十幾年來,多項研究已證實低溫療法對於心跳停止經急救復甦後仍昏迷的病 人,能改善神經學預後及提高存活率。2015年美國心臟協會、歐洲復甦委員會和國際 復甦術聯合委員會發表的心肺復甦與緊急心臟照護指引,將低溫療法列入心跳停止經 急救復甦後仍昏迷的病人之標準治療準則。低溫療法會造成生理學上的改變、影響藥 物動力學和藥效學,亦可能引起併發症。因此,低溫療法執行過程,應監測病人的生 命徵象、相關檢驗數值及觀察是否發生併發症,並且給予適當的處置。本文將針對低 溫療法的執行流程、對生理學、藥物動力學和藥效學的影響、可能造成的併發症及執 行時須監測的項目加以說明和探討。 |
| 英文摘要 | Over the past decade, clinical evidence have shown that therapeutic hypothermia for comatose patients with return of spontaneous circulation (ROSC) after cardiac arrest could improves neurological outcomes and survival rate. In 2015, cardiopulmonary resuscitation and emergency cardiovascular care guidelines published by the American Heart Association, European Resuscitation Council, and the International Liaison Committee on Resuscitation recommended that therapeutic hypothermia become a part of standardized treatment after resuscitation. Hypothermia may result in physiologic changes, pharmacokinetic and pharmacodynamic alterations, it may also cause some complications. It is important to monitor patient’s vital signs and laboratory test data closely, and manage complications properly during hypothermia treatment period. In this article, we will introduce the therapeutic hypothermia protocol, discuss the influence of hypothermia treatment on physiology, pharmacokinetics and pharmacodynamics. We also review monitoring and management of the complications during therapeutic hypothermia. |
