英文摘要 | Cardiac arrest secondary to hyperkalemia in severe renal failure is not uncommonly encountered in hospitalized patients. Even in cases of aggressive therapy for hyperaklemia and advanced life support, many patients still die from refractory hyperkalemia. We report on a 53-year-old non-oliguric uremic woman on regular hemodialysis that developed sudden cardiac arrest, secondary to severe hyperkalemia, with a plasma potassium concentration of 9.4 mmol/L. The cardiac arrest persisted after the initation of cardiopulmonary resuscitation and intensive treatment for marked hyperkalemia for 30 minutes. However, with the use of hemodialysis with low-potassium dialysate (1.0 mmol/L) during prolonged car-diopulmonary resuscitation, the patient’s cardiac rhythm was restored 20 minutes later. She recovered with-out neurological sequelae. We therefore suggest that hemodialysis should be considered as a therapeutic option during cardiopulmonary resuscitation for life-theatening cardiac arrest secondary to severe hyperkalemia. |
