中文摘要 | 背景:人工耳蝸植入兒童在其口語上仍有一定的限制,特別是在語調的表達,而華語為聲調語言,語調在表達語義時具有相當的重要性。故本研究旨在於探討人工耳蝸植入兒童在聲調及語調表達能力,並探討個體因素的差異對語調表達的影響。方法:收集人工耳蝸植入的先天性聽損兒童進行聲調聽辨、聲調表達及語調表達的評估測驗,施測時個案皆為國小高年級以上,長期穩定使用人工耳蝸,並以口語做為主要溝通方式,無合併其他身心障礙。採用音節聲調聽辨測驗、雙字詞聲調表達測驗及語句仿說測驗,分別評估聲調聽辨、聲調表達及語調表達能力。並以植入年齡與人工耳蝸使用時間,來探討可能影響語句仿說能力的相關因素。結果:共21名人工耳蝸植入兒童加入本研究,平均植入年齡3.11歲、施測時平均年齡12.03歲、人工耳蝸使用時間為8.92年。聲調聽辨平均正確率為79.37%,聲調表達平均正確率為95.24%,語句仿說平均相似度為94.39%;三項之間皆呈現顯著正相關(p < 0.01),其中聲調聽辨可以預測語句仿說相似度(p < 0.001)。植入年齡與人工耳蝸使用時間對於語句仿說相似度皆未達顯著相關。結論:先天性聽損孩童經長期使用人工耳蝸,在聲調聽辨、聲調表達和語調表達上皆有不錯的表現,且三者間皆有顯著之正相關。聲調聽辨可以預測語調表達的表現。臨床人員在評估或訓練華語聽損兒童的語調表達時,應考慮納入聲調聽辨及聲調表達之評估與訓練。 |
英文摘要 | BACKGROUND: Cochlear implants(CI) have assisted greatly in increasing language skills among prelingual hearing-impaired children. However, the verbal production of these pediatric CI recipients still have some restrictions, especially for speech intonation. This is particularly meaningful for Chinese-speaking people because Mandarin tones impart different meanings in addition to conveying emotion or implications. The purpose of this study is to investigate the correlation of tonal auditory perception, tonal production and the imitative production of sentence intonation. Their impact on speech intonation among cochlear-implanted children is discussed. METHODS: We recruited prelingually deaf children who had received CI in National Cheng Kung University Hospital to participate in this study. The demographic characteristics of the subjects were at least 5 grade students in elementary school, longterm CI use, verbal language as the primary means of communication and no other disabilities. The subjects were scored using three different tests for various auditory/ speech performance measurements. These included the "Mandarin Lexical Tone Recognition Test (MLTR)" from the "Tone Auditory Test Battery (MTATB)" for tonal auditory perception, "Chinese two-character words test" for tonal production and "Sentence imitation test" for the imitative reproduction of sentence intonation. Other variables, including age of implantation, duration of implant use, age at test, residual hearing level, use of hearing aid, sentence length and tone of last word in the sentence were also included and analyzed. RESULTS: Twenty-one prelingually deaf children joined voluntarily. They were implanted with CI at the average age of 3.11 years old, tested at the average age of 12.03 years old, and used a CI for the average of 8.92 years. The accuracy result of tonal auditory perception was 79.37%, the accuracy result of tonal production was 95.24%, and the result of the similarity of imitative sentence production was 94.39%. According to the Pearson product-moment correlation test, there was a significant positive correlation (p <0.01) for the three test results. Furthermore, results of tonal auditory perception test can predict the outcome of the imitative production of sentence intonation (p <0.001). The other explored factors, such as age of implantation, duration of cochlear implant use showed no significant correlations. CONCLUSIONS: This study concludes that prelingually deaf children can perform well in the areas of tonal auditory perception, tonal production and speech intonation when aided by long-term cochlear implants. Performance of tonal auditory perception can predict the outcome of the imitative production of sentence intonation. Therefore, we recommend clinical staff involved in the assessment or training of congenitally deaf Chinese children include tonal auditory perception and tonal production training to enhance the effects of speech intonation training courses. |
