中文摘要 | 簡介: 2009年的意外事故為國人第六大死因,每年死亡人數超過 7000人。本研究的目的是以北台灣創傷患者之入院前時間間隔來分析本區創傷醫療的現況。 方法:本研究追溯所有 2009年入本院之外傷患者。研究數據為外傷醫學會指定收集的參數。針對本區緊急醫療系統 (EMS)進行分析,評估院前時間 (pre-hospital interval)、現場停留時間 (on-scene interval)及送醫時間 (transport interval)是否影響病人的死亡率。 結果:針對本院 2009年共 1011位外傷患者進行研究,約 60%患者的受傷機制為道路交通事故, 4.7%為嚴重創傷患者,總死亡率為 3.1%。其中 404位 (40%)患者經緊急醫療系統 (EMS)入院,院前的時間約 23分鐘、現場停留時間約 9分鐘、送醫時間約 7分鐘。對於重大創傷的患者,送醫時間超過 5分鐘顯著增加病人的死亡率。 結論:本研究為北台灣院前創傷急救系統現況分析。各院前時間間隔符合國外相關研究結果。然而,本地區的數據顯示:延長送醫時間大於 5分鐘顯著的增加嚴重創傷患者死亡率。 |
英文摘要 | introduction: In Taiwan, accidents and adverse events represent the top 6th cause of death in 2009, and account for over 7,000 annual deaths. The objective of this study is to evaluate the rapidity of pre-hospital trauma care in our locality. methods: This is a retrospective study of all trauma patients admitted to our hospital in 2009. All data were prospectively collected based on parameters designated by the Formosa Association for the Surgery of Trauma (FAST). Patients transported by our emergency medical system (EMS) were analyzed for the pre-hospital, on-scene, and transport intervals. These intervals were then assessed for impact on patient mortality. results: A total of 1011 trauma patients were studied. This included 47 (4.7%) major trauma patients. Most of the injuries were RTIs (60%). Overall mortality was 3.1%. Only 40% (n=404) of the patients were brought by EMS. The median pre-hospital interval was 23 minutes. The median on-scene interval was 9 minutes, and the median transport interval was 7 minutes. For major trauma patients, transport interval over 5 minutes was associated with statistically significant increase in patient mortality. Conclusion: This is the first study illustrating the current pre-hospital trauma care system in Taiwan. Overall pre-hospital times are comparable to results previous reported. However, our data shows that prolonged transport interval (>5 minutes) among major trauma patients significantly increased mortality in our region and carries the potential for improvement. |
