| 中文摘要 | 背景和目的:人工植牙成為局部或全口缺牙病人熱門的治療方式之一。但是拔牙後齒槽骨的吸收往往造成植牙的困難度增加。因此拔牙後,齒槽骨保存術對於將來要植牙的區域是很重要的。而齒槽骨保存術關鍵點在齒槽骨內放入的移植骨材料,幫助減少齒槽骨吸收。要決定移植骨材料的成效便是利用動物模型製造骨缺損來測試。目前並無文獻提出兔子脛骨的絕對骨缺損的大小(絕對骨缺損的定義是在骨缺損內,新骨生成比例不能超過10%),所以本篇研究的目的就是要測試兔子脛骨的絕對骨缺損的真實尺寸。方法:本篇研究使用6隻紐西蘭白兔,測試兩種骨缺損,分別是8釐米及11釐米。隨機選取3隻兔子在脛骨上手術製造8釐米的圓形骨缺損,另外3隻兔子在脛骨上製造11釐米的骨缺損。之後分別在2、4及8週後將兔子犧牲,利用放射影像、組織學檢查來觀察圓形骨缺損骨生成的比例。結果:從放射影像學檢查顯示出8釐米及11釐米骨缺損都有骨生成。8釐米骨缺損相較11釐米有較大量的放射不透性物質生成。8週後,兩種骨缺損都可在放射影像及組織學檢查中觀察到新生成的骨小島。此外,11釐米骨缺損的新骨生成比例經計算超過70%。也就是不符合絕對骨缺損的定義。結論:本研究的結果顯示11釐米骨缺損仍然不足以當作兔子脛骨的絕對骨缺損的大小,但是在兔子脛骨不能手術製造大於11釐米,因為會造成骨頭斷裂。 |
| 英文摘要 | Background and purpose: Following tooth extraction, remodeling of the extraction socket results in a net shrinkage of the alveolar ridge. Socket and ridge preservation is very important if a dental implant is planned. The critical size defect (CSD) is used in experimental animal models to determine the effectiveness of newly developed biomaterials. The present study evaluated the spontaneous healing capacity of surgically created unicortical defects at different time points in order to determine the CSD of rabbit tibiae. Methods: Six New Zealand white rabbits were used in this study. Eight-millimeter defects were surgically created in the tibiae of each of 3 randomly selected rabbits, and 11-mm defects were created in another 3 rabbits. The defects were analyzed by means of radiographic, histologic, and histomorphometric analyses after the animals were killed at 2, 4, and 8 weeks after surgery. Results: The radiographic findings showed that the defects gradually closed over time. The 8-mm defects were filled with a larger amount of radiopaque substance. In addition, the 11-mm defects were also shown to have spontaneously healed at 8 weeks in the present study. Therefore, they did not satisfy the definition of a CSD because the percentage of new bone area exceeded 70%. The histomorphometric analysis suggested that it was impossible to determine the precise CSD of rabbit tibiae. Conclusions: The results of the present study suggest that 11 mm is an insufficient size for a CSD, and it may be impossible to create larger defects in rabbit tibiae. |
