| 中文摘要 | 本研究之目的主要評估採用影像導引放射治療病患擺位的不確定性,並估計治療所需的外擴邊緣及其影響因子。探討可能影響因子包含年齡、性別、治療部位和腫瘤分期、轉移程度、體質比。對於113位接受影像導引放射治療的病患,收集3725組位移修正資料進行分析。依治療部位分成頭頸部、胸腹部和骨盆部,計算位移平均值、系統誤差(Σ)和隨機誤差(σ)。利用van Herk所提公式(2.5Σ+0.7σ)計算在病患擺位的不確定性下所需的外擴邊緣。在頭頸部病患中,左右(X軸)、頭腳(Y軸)及前後(Z軸)三軸所需的外擴邊緣分別為4.48mm、5.01mm、3.82mm;在胸腹部為6.44mm、15.57mm、7.65mm;在骨盆部為6.37mm、15.15mm、8.66mm。利用複迴歸分析,我們發現腫瘤位置、年齡、性別為具有統計學上的顯著差異(p <0.01)之影響因子。治療胸腹部與骨盆部者X,Y,Z三輪外績邊緣皆需高於治療頭頸部者。同樣地,女性患者之X,Y二軸外擴邊緣需高於男性患者,而年齡愈高者之Y軸外擴邊緣則需較高。最後我們建議外擴邊緣設定值需依據治療部位、年齡及性別進行個別化調整。 |
| 英文摘要 | This purpose of this study is to evaluate the set-up uncertainty and prognostic factors of set-up margin for patients treated with image guided radiation therapy (IGRT). The factors includes age, gender, treatment site and stage, extent of metastasis、body mass index. This study analyzed 3725 correction shift data of 113 patients treated with IGRT. The mean displacements of translational shifts, population systematic errors (Σ) and random errors (σ) were calculated, and the set-up margins were approximately estimated. according to the van Herk's formula (2.5Σ+O.7σ), The set-up margins in the X (L-R), Y(S-I) and Z (A-P) axes were 4.48 mm, 5.01 mm, 3.82 mm for the patients treated at the head and neck site; 6.44 mm, 15.57 mm, 7.65 mm for those at abdominal site; and 6.37πun, 15.15 mm, 8.66 mm for those at pelvic site, respectively.Age, gender and treatment site were observed statistically significant correlated with the set-up uncertainty. Those treated at the abdominal or pelvic site needed more set-up margin extension in all the three axes compared with those at head and neck site Meanwhile, more extension of the set-up margin in X and Y axes were needed in female patients, so as the Y axis in those with older age. We concluded the design of the set-up margin extension should be individualized based on the treatment site, gender, and age. |
