中文摘要 | 電腦斷層血管攝影(CTA)多相(multi-phase)技術影像檢查,是肝臟CTA常用的檢查技術,利用晚期動脈相(late-arterial phase)、肝門靜脈相(portal venous phase)、延遲相(delayed phase)的比較,評估肝臟疾病及種類;高血管性(hypervascular)肝臟腫瘤經常在動脈相強烈顯影,在肝門靜脈相、延遲相隨之淡化,在多相技術CTA中,肝臟腫瘤動脈相對於診斷尤其重要,但在肝臟腫瘤動脈相CTA,我們必須很準確的一次就取得動脈相充分顯影且無門靜脈相,才可算是最理想的肝臟腫瘤動脈相掃瞄,所以掃瞄開始時間點的偵測,即顯得相當重要;利用多切面電腦斷層(MSCT)全自動藥劑濃度監控(automatic bolus tracking)控制開始的時間點,當監控位置有興趣的測量區(Region Of Interest; ROI)CT值到達預設的觸發閾值(trigger threshold)時才開始掃瞄;相當多藥劑濃度監控方面研究報告中,延遲時間(delay time)均列為最重要參數之一此次研究將較為不客觀的延遲時間降至最低,由全自動藥劑濃度監控控制開始掃瞄時間點,且盡可能在最短時間內完成掃瞄,排除延遲時間對於掃瞄的干擾,利用不同的藥劑濃度觸發閾值控制,取得肝臟腫瘤動脈相影像做比較,找出最理想的濃度監控閾值,即影像上高對比的時間,及分析統計延遲時間,以取得最佳的肝臟腫瘤動脈相影像,提供醫師診斷判讀。 |
英文摘要 | CTA of liver with multi-phase enhancement is used as an important routine examination. Application multi-phase (arterial, portal venous and delayed phase) of CTA evaluated kinds of hepatic disease. Hyper-vascular hepatic tumor is usually found as an early strongly enhanced nodule on arterial phase and rapid wash-out of contrast medium on portal venous and delayed phases. Otherwise we must accurately obtain proper arterial phase CT images and avoid too early or too late obtaining arterial phase of hepatic tumor CT images. So it is important to determine the timing of scan to get a proper arterial phase image. Automatic bolus tracking is used as a method to inspect the region of interest (ROT) in aorta as a trigger threshold, which is preset at a level of Hounsfield unit (HU). If it arrives at the level of threshold HU, the CT scan will be started to obtain arterial phase image. There are many reports about trigger threshold system and delayed scanning in multiphase hepatic CTA. The purpose of our study is to find a suitable and proper HU and high contrast timing of images as a trigger threshold without delay times of contrast medium after intravenous injection and to get a best arterial phase images for diagnosis. |
