中文摘要 | 本研究主要的目的是建構短版的幻聽評估量表。Frederick發展32題幻聽評估量表,經由原作者同意後,江、楊將其中文化,並經過初步的信效度檢定,也應用於某些研究,但是對於慢性思覺失調症個案而言,32題的幻聽評估量表仍有些負荷。針對北部地區精神科兩家醫療院所的日間留院與慢性病房治療的慢性思覺失調症並有幻聽干擾的病人共209位,填寫長版幻聽評估量表。研究者以臨界比(critical ratio)進行初步題項刪除;並以原始向度架構,採用迴歸分析的前進選擇法逐一選擇三向度的題目,針對每一向度逐步的加題直到短版與原始版內在一致度無差異,因素分析顯示KMO值為.877,可解釋的變異量為57.11%;原始/短版三個向度的題數(內在一致度)分別是:幻聽特性8題(.85)/6題(.84)、幻聽信念7題(.84)/3題(.82)、幻聽的情緒與行為結果17題(.91)/10題(.89),目前短版題數為19題,未來有必要再發展比19題更少的題目,以利護理人員了解慢性思覺失調症病患的幻聽特性、幻聽信念與幻聽情緒行為結果。 |
英文摘要 | Background: The original 32-item Auditory Hallucination Assessment Scale (AHAS) was translated into Chinese by our research team which had been used to measure auditory hallucination for schizophrenia patients. However, it was suggested that the lengthy questionnaire should be modified to reduce participants' burden of answering questions and a simple form should fit better to psychiatric and research settings. Based upon the advice, we have tried critical ratio to eliminate some questions from the initial lengthy form of Auditory Hallucination Assessment Scale (AHAS-LCF). The purpose of the study was thus aimed to develop a simple Chinese form for Auditory Hallucination Assessment Scale (AHASSCF) and to further enable it a reliable and valid test for schizophrenia patients in Taiwan. Methods: The participants in research were 209 schizophrenia hospitalized patients at the average age of 41.89 from Northern Taiwan. We adopted a stepwise regression with forward procedure to select testing items. The dependent variables were the overall scores on the lengthy-form of AHAS (AHAS-LCF). The independent variables were the scores for each item in the AHAS. All of the testing items were included in regression models in which a forward stepwise procedure was used to identify the best predictors of the total scores. We repeated the procedures and analyzed their internal consistency until both of AHASLCF and AHAS-SCF displayed equivalent. The factor analysis then proceeded to identify the domains of AHAS. The internal consistency of AHAS-SCF was evaluated by using Cronbach's α., and the test-retest reliability was evaluated by intra-class correlations and Spearmen correlations. Results: The overall score of short form AHAS and its dimensions were correlated with the long form's (all r>0.951, p<0.001). The EFA turned out three domains with eigenvalues >1, explaining 37.40, 11.90 and 7.75% of the variance, respectively, accounting for a total of 57.11% of the variance. The three domains were given names respectively: 'characteristics' (6 items), 'beliefs' (3 items), and 'emotional-behaviors' (10 items). The total 19 items on the AHAS-SCF showed internal consistency (Cronbach's α) >.820; the test-retest reliability run by Intra-Class Correlation (ICC) was .98, and the Spearmen Correlation .96 for the total AHAS score. Conclusions: The research results proved that the AHAS-SCF can be properly used for assessment of schizophrenia patients in Taiwan. |
