中文摘要 | 隨著年齡的增長,高齡長者罹患憂鬱症比率亦不斷上升。本文針對音樂治療運用於憂鬱長者的相關研究加以整理,以期望透過實證文獻探討的過程,瞭解音樂治療對老年長者憂鬱改善成效。資料搜尋來源為PubMed、Cochrane Library、ProQuest、EBSCO及CEPS中文電子期刊等資料庫。關鍵字使用高齡(elderly)、音樂(music or music therapy)和憂鬱(depression)。搜尋近五年(2009至2013年)的文章,共得27篇。在選取符合篩選標準並排除重複出現後,找出10篇音樂治療之臨床試驗研究,進行文章的分析與統整。結果指出音樂治療能改善長者憂鬱程度,且達統計顯著性差異;在研究方法上,國內外學者多隨機控制實驗或類實驗方式以進行研究;個案來自於門診、長期養護機構或社區;在音樂介入內容方面,國內音樂治療多屬於動態式(身體律動、肢體運動),而國外音樂治療多屬於靜態式的(聆聽古典音樂、柔和音樂);音樂介入時間約20-40分鐘/1次、頻率從每週1或2次到每天1次、持續2-18週不等。然而,一些研究限制卻無法避免,如樣本數過少、缺乏診斷憂鬱症診斷標準,或實證等級良好的研究文章不足。因此,未來仍需要更多研究證實音樂治療對憂鬱老年長者的成效。 |
英文摘要 | Depressive symptoms have been reported as increasing with advancing age, and music therapy may help ease the symptoms of depression. However, the effectiveness of music therapy in elderly depression remains inconclusive. In the paper, related previous studies published from 2009 to 2013 were summarized and reviewed. The following databases were used to identify related literature: PubMed, Cochrane Library, ProQuest, EBSCO and CEPS. The search based on the terms "elderly," "music or music therapy," and "depression" found a total of 27 publications. After excluding duplication, 10 publications were identified for analysis in this review article. Most of them were randomized controlled studies or studies with quasi-experimental designs. Music intervention in Taiwan tends to focus on rhythmic movements of limb or body, while its counteparts in other countries usually involves listening to classic and soft music. Music intervention in general lasts approximately 20-40 min per session and is administered for 2 to 18 weeks based on a frequency ranging from one session per day or 1 to 2 sessions per week. In spite of existing research limitations, notably small sample size, absence of a uniform standard for the diagnosis of depression, and lack of evidence-based studies, the elderly with depression seem to have derived benefits from music therapy. In conclusion, although music therapy appears to be capable of mitigating elderly depression, its effectiveness needs to be further supported by more studies in the further. |
