中文摘要 | 背景及目的:全民健康保險給付制度不斷更新,特別是總額預算後,臨床護理人員的工作量及工作壓力增加,容易導致工作倦怠及有離職的打算。本研究目的為探討在健保總額預算期間,臨床護理人員的工作倦怠、離職意向,及兩者的影響因子。方法:採用橫斷式問卷調查,對桃園縣臨床護理人員以分層比例隨機取樣,邀請臨床護理人員填寫問卷。以工作倦怠-醫療行業調查(MBI-HSS)(含三個向度:情緒耗竭、去人性化、個人成就)來測量工作倦怠。結果:2006年7-12月得有效問卷374份(48.5%)。情緒耗竭的平均數為27.43(百分位=50.8)、去人性化為8.72(百分位=29.1)、個人成就為34.06(百分位=71.0)。多變量分析結果顯示,醫療工作的壓力源感受(p<0.0001)、支持性因應(p<0.0001)、年齡(p<0.0001)及健保總額預算前後工作感受改變(p<0.0001),與情緒耗竭有統計顯著關係。年齡(p<0.0001)、支持性因應(p<0.0001)、醫療工作的壓力源感受(p<0.0001)、聘僱方式(p=0.0067)與去人性化有統計顯著關係。控制性因應(p<0.0001)、年齡(P<0.0001)與個人成就有統計顯著關係。高達249位(67.0%)臨床護理人員有離職意向。多變量分析結果顯示,健保總額預算前後工作感受改變(p=0.0039)、醫療工作的壓力源感受(p<0.0001)、情緒耗竭(p<0.0001)、個人成就(p=0.0161)與離職意向有統計顯著關係。結論:總額後臨床護理人員工作倦怠的情緒耗竭嚴重,2/3有離職意向。 |
英文摘要 | Background and purpose: Due to frequent change of the payment system of Taiwan National Health Insurance (TNHI), especially after global budget, burnout and intention to quit their job is resulted from increased workload and job stress among clinical nurses. In this study, we investigated the magnitude of job burnout and quit intention, and analyzed factors associated with burnout and quit intention among clinical nurses during the implementation of the global budget payment system of the TNHI. Methods: A cross-sectional study with a stratified and proportional random sampling was used to obtain the study sample in Taoyuan, Taiwan. The Maslach Burnout Inventory-Health Service Survey (MBI-HSS), including three constructs (emotional exhaustion, depersonalization, personal accomplishment), was used. Results: From July to December 2006, 374 valid questionnaires (48.5%) were obtained. The mean score of emotional exhaustion was 27.43 (percentile=50.8), depersonalization was 8.72 (percentile=29.1), and personal accomplishment was 34.06 (percentile=71.0), respectively. Multivariate analysis reveals that medical workers’ stress (p<.0001), supported coping (p<0.0001), age (p<0.0001), and work perceptions about the global budget for medical employees (p<0.0001) was significantly associated with emotional exhaustion. Age (p<0.0001), supported coping (p<0.0001), medical workers’ stress (p<0.0001), employment status (full time vs. part time)(p=0.0067) was significantly associated with depersonalization. Control coping (p<0.0001), and age (p<0.0001) was significantly associated with personal accomplishment. 249 (67.0%) of clinical nurses had an intention to quit their job. Multivariate analysis reveals that work perceptions about the global budget for medical workers (p=0.0039), medical workers’ stress (p<0.0001), emotional exhaustion (p<0.0001), personal accomplishment (p=0.0161) was significantly associated with quit intention. Conclusion: During implementation of global budget payment system, there was a serious problem of emotional exhaustion among clinical nurses and two-third of the clinical nurses had an intention to quit their job. |
