| 中文摘要 | 本文探討一位初診斷肺癌之中年婦女,原本因肺炎入院治療,但住院過程中,因症狀持續,須接受進一步檢查,檢查過程中非常焦慮,而且檢查結果竟然發現是肺癌,震驚之餘,個案無法面對癌症與相關治療,初期出現沉默,後來無望感受加重甚至有自殺想法,因此引發筆者想要進一步探討之動機。筆者於2009年2月27日至2009年4月10日照護個案,利用Gordon十一項健康功能評估工具評估個案,藉由實際照護、病歷檢閱、觀察、傾聽及身體檢查與評估技巧收集資料,分析歸納確立個案身、心問題有體溫過高、焦慮與無望感。筆者發現個案因無法面對初診斷癌症之衝擊,情緒波動起伏大,護理過程中透過主動關懷提供治療資訊、傾聽並轉移病患負向的想法與感受,增加陪伴時間、與個案建立互信關係,最後提供安全舒適環境,以減輕個案對癌症與治療的恐懼與不安,加上醫療團隊及病患友人的陪伴支持下,使個案能勇敢面對肺癌及後續治療,並達到最佳身心靈舒適之狀態。 |
| 英文摘要 | The purpose of this article is to describe a clinical experience in which a patient with lung cancer was treated. This patient’s initial hospitalization was for pneumonia management; however, after series of medical treatments, the middle-aged woman’s condition continued to deteriorate, rather than improve. Her doctor then arranged a series of medical tests to confirm the diagnosis. The patient was very anxious while waiting for the results of the tests. When the test results came back, they revealed that the patient had lung cancer. At first, the patient emotionally denied the diagnosis. Her response to hearing the diagnosis and its possible treatments was silence. She then went on to develop suicidal thoughts due to feelings of hopelessness. Because of her psychological issues that hindered her ability to cope with cancer, the author decided to document the experience of helping this patient. The author was her primary nurse from February 27 to April 10, 2009. After employing Gordon’s 11-item health assessment, care plans for this patient were drawn up. Daily nursing activities, information gathered from the patient’s medical chart, clinical observations, the patient’s responses to questions, and results from the patient’s physical assessment showed that the patient suffered from a high body temperature, anxiety, and feelings of hopelessness. Furthermore, the author identified that this patient had fluctuations in mood and was unable to effectively cope with cancer. The author thus set the nursing goals to be to ease the patient’s fear and discomfort from cancer in order to help the patient effectively cope with the disease, to follow the prescribed medical treatments for cancer, and to provide physical, psychological, and spiritual comfort. Nursing care plans for the patient included offering relevant information about lung cancer to the patient, listening to the patient and distracting her from her negative thoughts, establishing a trust relationship with the patient, and providing a safe and comfortable environment for her. |
