| 中文摘要 | 本文探討一位初診斷胰臟癌末期壯年男性面對生理不適並擔心一家老小, 嚴重產生無望感及死亡焦慮之護理經驗。護理期間為 2021 年 9 月 24 日至 2021 年 10 月 13 日,運用直接照護、觀察、傾聽、會談及運用 Gordon 十一項健康功 能型態為評估工具,確立個案有疼痛、無望感及死亡焦慮之健康問題。在照護 過程中,筆者與個案及其照護者建立良好互信關係,以放鬆技巧及配合藥物等 方式緩解疼痛不適,經跨團隊合作引導個案及家屬坦然面對癌症,說出彼此內 心感受,帶領家屬進行生命回顧,肯定自我存在價值,緩解個案對死亡焦慮, 維持應有生活品質及生命尊嚴。建議未來應持續推廣安寧照護之在職教育,輔 導護理人員照護癌末病人的知識與技能,提升臨終病人之生活品質,期望藉由 此護理經驗之分享能夠提供護理同仁往後照護類似個案之參考。 |
| 英文摘要 | This article discusses the nursing experience of a middle-aged man newly diagnosed with pancreatic cancer in the terminal stage of his life, faced physical discomfort, worried about his family, and developed a severe sense of hopelessness and death anxiety. The nursing period was from 24th of September to 13th of October in 2021, and the health issues encountered include pain, hopelessness and death anxiety were identified by using direct care, observation, listening, talking, and applying Gordon's 11 Functional Health Assessment as assessment tools. In the process of care, the author established a good relationship of mutual trust with the case and his caregivers, relieved pain and discomfort through relaxation techniques and medications; guided the case and his family to face cancer calmly; expressed each other's inner feelings; led the family reviewing their lives; affirmed the value of selfexistence; alleviated the patient's anxiety about death; and maintained the quality of life as well as dignity. It is suggested that in the future, on-the-job education in palliative care can be promoted continuously, and nursing staff counseling implement can upgrade the knowledge of caring for patients with terminal cancer to improve their quality of life. Hopefully by sharing this nursing experience, it will provide nursing colleagues as a reference for caring for similar cases in the future. |
