中文摘要 | 本篇個案報告主要是描述一位肝癌末期的個案,除須調適癌症所帶來的生理不適外,還需面對瘖啞溝通不良的案妻及家庭不睦等問題,凡此種種均讓個案倍感壓力,因此也產生了許多照護問題。個案護理期間為2008年8月15日至8月30日,筆者以直接照護、觀察等技巧收集資料,並藉由Gordon健康功能型態進行評估,發現個案有體液容積過量、疼痛及照顧者角色緊張等三項主要的健康問題。在護理過程中,筆者以持續性照護及主動關懷與個案、家屬建立良好的護病關係,並運用個別性的護理措施介入,處理個案的生理問題。此外,亦藉由護理專業的知識,不斷耐心教導瘖啞案妻各項照護技巧,使個案能獲得妥善的照護;另外,筆者亦擔任家屬間的溝通橋樑,讓家屬彼此達成共識,以期讓個案能在殘餘的歲月中,平靜的度過餘生。筆者希望藉由本篇個案報告,期能提供臨床同仁日後照護此類個案之參考。 |
英文摘要 | This article describes the nursing experience of a patient in the terminal stage of hepatocellular carcinoma whose physical discomfort and poor communication with his deaf-mute wife caused him much stress and led to many care issues. Nursing care was provided from August 15th to 30th, 2008. The authors used observations, interviews and Gordon's functional health pattern model to gather and analyze information. The results revealed three major health problems: excessive fluid volume, pain, and caregiver role strain. During the nursing process, the authors used continuous care and caring attitude to develop a good nurse-patient relationship with the patient and the family. Individual measures of nursing intervention were also adopted to deal with physiological problems, and by teaching the deaf-mute wife professional patient-care-skills, the patient was able to receive better care. The authors also serve as a bridge of communication to promote consensus between family members, allowing the patient to live his remaining years in peace. The authors wish to report this clinical case for future references. |
