中文摘要 | 氣喘的氣道重塑(airway remodeling)現象產生數種組織的改變,包含上皮細胞化生、支氣管平滑肌肥厚、上皮下纖維化及血管增生等。呼吸道的發炎細胞及組織細胞所分泌的變形生長因子(TGF)-β在氣喘的呼吸道中大量增加並可誘發呼吸道上皮細胞的凋亡 。TGF-β造成杯狀細胞(goblet cells)增殖與黏液的過度分泌有關,其產量也與上皮下纖維化的程度成正比。在上皮下纖維化的發展過程中,TGF-β可誘發纖維芽細胞(fibroblasts)分裂及分化成肌纖維芽細胞(myofibroblasts),並產生細胞間質蛋白。TGF-β可增加呼吸道平滑肌細胞的增殖及存活,進而造成支氣管平滑肌的肥厚。此外,TGF-β亦可誘發血管內皮細胞生長因子(VEGF)的釋放,促成血管重塑現象。阻斷TGF-β的活性可同時抑制氣喘中上皮細胞脫落,黏液過度分泌,血管增生及上呼吸道平滑肌肥大增生等現象。因此,減低及控制TGF-β的產量及作用應是對氣喘中之氣道重塑現象的治療是具意義並有助益的。 |
英文摘要 | Airway remodeling in asthma is defined by several structural changes including epithelial cell metaplasia, an increase in peribronchial smooth muscle mass, suepithelial fibrosis and angiogenesis. Transforming growth factor (TGF)-β released from inflammatory and structural cells in the airway is considered to play a pivotal role in the development of remodeling. Elevated levels of TGF-β have been found in the asthmatic airway and induce apoptosis in epithelial cells. Enhancement of goblet cell proliferation by TGF-β suggests a role in mucus hyper-secretion. Elevated levels of TGF-β correlate with subepithelial fibrosis. TGF-β induces proliferation of fibroblast cells and their differentiation into myofibroblasts and extracellular matrix protein synthesis during the development of subepithelial fibrosis. TGF-β increases proliferation and survival of airway smooth muscle cells, leading to the increased thickness of airway tissue. TGF-β also induces the release of vascular endothelial cell growth factor, contributing to the vascular remodeling in the asthmatic airway. Blocking TGF-β activity inhibits epithelial shedding, mucus hyper-secretion, angiogenesis, airway smooth muscle cells hypertrophy and hyperplasia. Reduction of TGF-β production and control of TGF-β effects would be beneficial in the development of therapeutic intervention for airway remodeling in asthma. |
