英文摘要 | Background: Malnutrition is closely correlated with morbidity and mortality in maintenance hemodialysis (MHD) patients. To analyze the correlations between food intake and nutritional status, we perform a prospective study in a single hemodialysis center. Methods: In this study, 87 MHD patients were enrolled. 24-hr dietary recall of all patients was performed by a single dietician within 1 week. The dietary components, including carbohydrate, lipid and protein, were then analyzed. A mid-week fasting predialytic blood sampling and post-dialysis SGA and anthropometric evaluation of every patient were performed. Results: The mean age of subjects was 59.5±11.8 years, 58.7% of them were male. The average total energy intake was 26.7±6.6 Kcal/Kg/day, and protein ingestion was 1.0±0.3 g/Kg/day. The average dietary components of all 87 patients showed carbohydrate 51.8±10.0%, lipid 32.8±8.3%, and protein 15.6±2.9%. The prevalence of malnutrition (defined as SGA≤5) was 32.2%. The total energy and protein intake were higher in well-nourished patients, but not for carbohydrate or fat intake, than those of malnourished patients, p<0.01. Moreover, high sensitive C-reactive protein (hs-CRP) was higher in malnourished patients. Among the anthropometric evaluations, only body mass index (BMI), mid-arm circumference (MAC) and mid-arm muscle circumference (MAMC) were found to be significantly higher in well-nourished patients. Conclusions: Malnutrition is common in MHD patients. Well-nourished patients have much higher total energy and protein intake than malnourished patients. Acute inflammatory status is associated with malnutrition. Muscle mass measurements such as BMI, MAC and MAMC give better correlations with nutritional status than fat measurement. |
