中文摘要 | 本研究主要探討肥胖者體重下降對血清瘦體激素(leptin)濃度和胰島素抗性的影響。實驗以35位減重班無糖尿病及糖尿病家族史,其體重均超過理想體重(ideal body weight, IBW)120%以上之學員為對象(肥胖組),給予營養教育和運動的四個月減重課程,並以40位體重介於90%至110%IBW的健康者作比對(控制組),觀察受試者的體重、身體質量指數(body mass index, BMI)、空腹血漿葡萄糖值、血清胰島素濃度、胰島素抗性之恆定模式的評估方法(Homeostasis model assessment method – insulin resistance index, HOMA-IR index)與血清瘦體激素濃度的變化。結果顯示,不論是空腹血漿葡萄值、血清胰島素濃度、HOMA-IR指標或血清瘦體激濃度,肥胖組均比控制組顯著增高(p<0.05),肥胖組的BMI與體重下降後,其空腹血漿葡萄糖值、血清胰島素濃度、HOMA-IR指標和血清瘦體激素濃度也明顯降低。綜合以上結果得知,肥胖者體重下降會降低空腹血漿葡萄糖值、血清瘦體激素濃度和胰島素抗性。 |
英文摘要 | This study investigated how body weight reduction alters serum leptin concentrations and insulin resistance. Thirty-five obese subjects ( > 120% of ideal body weight) were recruited. All of them were non-diabetics with no diabetic family history. In comparison with healthy individuals who had normal weight (n=40), our results showed that the obese group had higher levels of fasting plasma glucose, serum insulin, homeostasis model assessment method for insulin resistance index (HOMA-IR index), and serum leptin (p<0.05) than the control group. A weight reduction program for the obese group was carried out for 4 months, They all received a full course of nutritional education are exercise. Their fasting plasma glucose, serum insulin, HOMA-IR index, and serum leptin levels significantly improved (p < 0.05) after BMI reduction (BMI: 30.2 ± 2.4 vs 28.2 ± 2.5 kg/m2, p<0.05). In conclusion, body weight reduction decreases fasting plasma glucose, serum leptin concentrations, and insulin resistance. |
