中文摘要 | 本篇個案報告運用Watson關懷理論照顧一位重症患者之主要照顧者護理經驗。護理期間自民國2006年4月18日至2006年5月10日,筆者於外科加護病房照護因車禍遭嚴重頭部外傷昏迷之案女,護理過程中運用Watson關懷理論技巧與個案建立良好互信關係,藉由觀察、會談、電話訪談、身體評估等方式收集資料,發現個案主要健康問題有焦慮、預期性哀傷、照顧者角色緊張及家庭因應能力失調:妥協性等,透過護理措施介入,鼓勵家屬分享內心感受,利用溝通技巧肯定接受正向和負向的感覺,提供社會有效資源及居家護理師轉介,以減輕身心、社交、經濟等壓力,使主要照顧者強化適應性因應技巧,進而促進個案自信及耐受力。期望藉此照護分享,勉勵護理人員除了病患還包括家屬身、心、靈性層面之照顧,以達到全人照顧的基礎。 |
英文摘要 | This case report described the author's nursing experience of application Watson caring theory care with a primary caregivers who cares a patient with head injury and coma. During the period from April 18 to May 10, 2006, The author nursing the caregiver's daughter who suffered from abrupt traffic accident to make severe head trauma and coma in surgery intensive care unit. The period of nursing care was application Watson caring theory address and build believe in well association by case, The main health problems were identified through observation, interview, telephone interviews, physical assessments, and other information-gathering methods, The addressed nursing diagnosis werer: anxiety, expectations of sorrow, the role of caregivers and family tension response capacity imbalance: com promise. Nursing intervention such as to encourage families to share feelings, and use communication skills to approve the positive and negative sense, to provide the social resources and home care nurse referral to reduce physical, psychological, social, economic and other stress, to make the caregivers can strengthen adaptive response skills, to go a step further promotes self-confidence and tolerance. The author hoped this experience might be shared and encouraged the nurses in addition to the patients also include the families's physical, mental and spiritual dimension of care, to achieved all-round care foundation. |
