| 中文摘要 | 本文是描述一位罹患口腔癌末期病患因呼吸道阻塞行永久性氣切造口術之住院照護經驗，個案在此次住院期間發疾病已轉移、惡化，必需依賴呼吸器維持呼吸功能；而病患與家屬必也須共同面對疾病之變化。護理過程中，筆者藉由身體評估、會談與出院後電院訪談等方法收集資料，並以Gorden十一項功能健康型態評估，確立健康問題為疼痛、口腔黏膜改變、組織完整性受損及身體心像改變。針對個案健康問題提供適當護理措施，如疼痛處置、口腔清潔、腫瘤傷口護理以及協助病人調適外觀的變化等。經由人際關係的建立，促進個案對護理人員之信任與配合；不斷給予病人及家屬心理層面的支持鼓勵，使個案及家屬能正向面對疾病所帶來的改變。此外，評估個案與家屬的出院準備需求，協調轉介轉機構，希望在個案生命的最後階段獲得較佳的生活品質。 |
| 英文摘要 | In this case study we report our nursing experience caring for a terminally ill oral cancer patient suffering metastasis and aggressive cancer disease. He had received a tracheotomy and was put on a ventilator to resolve an airway obstruction and maintain his gression of the disease and concurrent physical changes. While caring for this patient, the patient’s medical data were collected and nursing problem accessed using Gorden 11 functional health patterns by both bed-side interview and post-discharge telephone interview. The patient’s nursing problems were pain, impaired oral mucous membrane, impaired tissue integrity and disturbed body image. The physical interventions included pain management, oral car and tumor fungating wound care. The psychiatric interventions included increasing trust and improved coordination with the patient and his family member. The spiritual nursing intervention involved supporting and encouraging the patient and his family to face the progress of the disease positively as defined by hospice palliative nursing theory. Finally, the author assessed the needs of the discharged patient and his family, negotiated and transferred the patient to an institution that ensured a high standard of care for this patient and his family members until his death by this disease. |
