| 中文摘要 | 背景:在執行氟-18-去氧葡萄糖(FDG)全身正子斷層掃描(PET)的受檢者,發現位於下腹部皮下組織有FDG攝取增加的疑似腫瘤,以PET/CT之CT(computed tomography)影像比對,偶爾發現有低密度的腫塊,經病史的詢問及臨床的表徵,受檢者皆曾有出國植入胎盤抽取的外來物,因此將案例整理加以分析。 方法:收集植入胎盤抽取物的受檢者人數共9人,其中7例為女性、2例為男性,年齡範圍46-67歲,平均年齡55歲。每位受檢者注射FDG,使用PET/CT掃描,測量受檢者植入胎盤抽取物在CT影像上的大小及CT number(Hounsfield units; HU),以及相對位置PET影像的SUV值加以對照。 結果:9位受檢者共有24個植入點,最大有2.3×1.5公分,standard uptake value (SUV)可高到2.58,CT HU值介於6至34之間。其中一位案例有六個胎盤抽取物植入點,第一點距離PET檢查當天時間分別為8天,CT HU值33,SUV值1.07;第二點距離PET檢查時間1個月,CT HU值17,SUV值2.57;第三點距離PET檢查時間2個月,CT HU值25,SUV值1.22;第四點距離PET檢查時間3個月,CT HU值23,SUV值1.7;第五點距離PET檢查時間4個月,CT HU值23、SUV值1.2;第六點距離PET檢查時間5個月,CT HU值15,SUV值0.6。 結論:本研究發植入胎盤抽取物無論從PET或CT影像上確實很難排除腫塊之可能。外來異物植入所引發的FDG攝取會隨著個體自體免疫的反應及植入時間而有不同的表現,在PET FDG攝取SUV值呈現複雜變化。本研究以PET/CT影像提供診斷胎盤抽取物植入體內所引發的免疫發炎反應的參考。 |
| 英文摘要 | Backgrounds: In 18F-FDG PET/CT scan, suspicions tumors with increased 18F-FDG uptakes are rarely found in the subcutaneous tissue of lower abdomen. According to the examinee’s medical history and clinical appearance, this kind of low-density mass shown in CT image are later found to be foreign implantation of placenta extract. Methods: Nine examinees (7 female, 2 male; age 46-67 year-old with a mean of 55 year-old) with implantation of placenta extract were recruited in this study. All patient received FDG PET/CT scans. Measurement about size and CT number (Hounsfield units; HU) of placenta extract in the CT images was performed for each patient. Correlation with the SUV value of corresponding site in the PET images was also done. Results: Twenty-four implanted points were found in these nine examinees. Size of the largest implant was 2.3×1.5cm. The standard uptake value varied and could be as high as 2.58. CT (HU) ranged from 6 to 34. Particularly in one case, 6 placenta implanted points were observed and they were implanted eight days, one month, two months, three months, four months, and five months before PET/CT scan with CT HU 33/SUV 1.07, CT HU 17/SUV 2.57, CT HU 25/SUV 1.22, CT HU 23/SUV 1.7, CT HU 23/SUV 1.2, and CT HU 15/SUV 0.6, respectively. Conclusion: In this study, we found that it is truly difficult to differentiate foreign placenta implantation from tumors. In response to immune reaction of individuals and duration of implantation, diverse SUV values of hypermetabolism caused by foreign implants are observed in this study. Using PET/CT scan, this article provides imaging information concerning the placenta extract implantation and immunological inflammation reaction. |
