英文摘要 | Pathological features of the stone-bearing intrahepatic bile ducts in hepatolithiasis, are reviewed with an emphasis on the mucin profile and bile duct fibrogenesis. A majority of stones in this disease are calcium-bilirubin stones, while cholesterol stones are also increasing in Japan. The affected intrahepatic bile ducts bearing the former stones are fibrotic and their lumen shows alternative dilatation and stenosis. Histopathologically, the affected bile ducts show chronic cholangitis with hyperplasia of the intrahepatic peribiliary glands and biliary lining epithelia, bile duct fibrosis and nonspecific inflammatory cell infiltration in a variable combination (chronic proliferative cholangitis). Biliary mucosa of the stone-containing ducts show mucus gland hyperplasia resembling pseudopyloric gland hyperplasia and mucin phenotypes compatible with gastric mucosa. These glands and also the hyperplastic surface epithelia secrete much acid and neutral mucin. Peribiliary mast cells expressing fibrogenic factors such as basic fibroblast growth factor and histamine may be significantly involved in bile duct and periductal fibrosis. infrequently, this disease is associated with cholangiocarcinoma which develops through a multi-steps process. Sequential phenotypic changes during carcinogenesis are being evaluated, although an underlying genetic alterations remain unclarified. It is important to evaluate the mechanism and process of stone formation, biliary fibrosis and carcinogenesis in hepatolithiasis by the modern technology available. |
