中文摘要 | 血管通路感染對長期規則血液透析病患而言是造成罹病率及致死率的重要原因。血管通路一旦遭金黃色葡萄球菌感染,由菌血症而來的轉移性感染並不少見。我們報告一位接受血液透析六年之病人發生對盤尼西林有抗藥性之金黃色葡萄球菌(methicillin-resistant Staphylococcus aureus MRSA)人工動靜脈瘻管感染和敗血症合併腰部脊椎之轉移性感染導致脊髓管狹窄。病人同時發生右側腰大肌膿瘍。此種特別的合併感染先前並未報告過。經由適當抗生素及外科介入治療,病人的感染得以改善。對規則血液透析病患而言,MRSA菌血症是一個嚴重的問題,當病人出現不可預期或不常見的症狀時,轉移性感染就必須列入考慮。 |
英文摘要 | Vascular access infection is an important cause of morbidity and mortality in patients with end-stage renal disease on maintenance hemodialysis (HD). When the access site is infected with Staphylococcus aureus, metastatic infection often results from bacteremia. We report a patient on maintenance HD for 6 years who developed a methicillin-resistant S. aureus (MRSA) graft infection with MRSA septicemia and metastatic infection of the lumbar spine, leading to spinal stenosis. He also had an abscess of the right psoas muscle. This particular combination of infected sites has not been previously reported. His infections resolved with appropriate antibiotics and surgery. MRSA bacteremia is a serious problem in patients on maintenance HD, and metastatic infection should be considered when the patients report having unexpected or unusual symptoms. |
