中文摘要 | 人口老化、慢性疾病與家庭結構變遷等現象,帶來長期照護的殷切需求。長期照護型態有居家照護、社區式照護與機構式照護等三種,其中機構式照護已成為現今長期照護體系中重要服務模式。本研究以台中縣現有接受長期照護機構照護的病患或家屬為訪視對象,目的在探究病患對機構照護服務品質之滿意情形以及入住後自覺健康改變狀況。本研究以經專家評估、預試後修訂之問卷做為調查工具,於民國93年(2004年)2~3月間,在臺中縣同意訪視的10家立案長期照護機構中,隨機抽取350名病患或其家屬進行問卷調查,所得有效樣本共220位,回收率62.86%。 統計結果顯示:受訪者以男性、已婚、小學學歷者居多,年齡在60歲以上超過了八成(82.8%),大多來自醫院轉介,入住時間超過一年者將近五成(45.5%),其日常生活功能傾向於少部分或完全依賴輔助者之情況。至於受訪者對長期照護機構內設備之滿意度是較高的,對硬體、軟體設備滿意情形方面,受訪者分別對「浴室/廁所地板防滑墊之安全性」、「定期血壓、血糖測量服務」滿意度最高;對硬體設備較不滿意的是「非復健用途之一般運動器材使用」、「機構內部裝潢及陳設氣氛」與「室內空氣品質」等項目,而對軟體設備不滿意者多集中在「現行本院每月照護收費標準」、「工作人員配置數量」、「院方提供早午餐及點心的選擇性」方面,尤其對「照護收費標準」不滿意者遠超過其他各項目,其滿意度平均值只有2.95分。另外,病患住進機構後多覺心情變愉快、也非常滿意其生理健康改善的情形。進一步檢測,個人基本變項與其對該照護機構滿意程度的差異,發現:不同性別、年齡之病患對於硬體設備滿意度有顯著差異(p<0.05)。而不同性別、年齡和婚姻狀況病患對機構所提供軟體設備的滿意度亦呈現顯著差異(p<0.05)。 本研究建議機構式照護機構宜將加強內部裝潢陳設、提供適質適量的一般運動器材,並監測照護機構之室內空氣品質等措施,列為短程經營改善重點,可彰顯其具體效益;進而將人員配置、餐飲選擇性與收費金額等軟體品質提昇,作為中長程持續討論修正的主要依據。期許照護機構經營者能針對不同特質如:性別、年齡等個別差異之病患需求,在有限成本下,發揮智慧與巧思,同中求異地滿足不同層次之照護需求,方能真正落實衛生及福利政策中對弱勢團體照護的目標,使機構式照護病患能直接感受到環境改善的效益,獲得更適切的機構式照護品質。 |
英文摘要 | The Department of Health in Taiwan, it divided the Long Term Care types to the following three types----The Home-based, the Community-based and the Institution-based types. The Institution-based type became the most important service pattern by now. The purposes of this research are to understand what do the residents' thinking on the service qualities of the long term care facilities in Taichung County region. This research referred to the quality control theories from the domestic and foreign literatures. Our questionnaire,the study tool,it was used in this survey which began from 2004 February to March. And the 350 samples were obtained from several nursing homes or long term care centers in Taichung County by random. After deleting the uncompleted ones, our effective sample size are 220, the return-ratio is 62.86%. The following items are our main outcomes---- About the residents and their family members’degrees of satisfaction on the organization hardware and software equipment items which they were stayed. Most of the samples satisfied in the hardware and software equipment items. The highest satisfaction score came from "the security of the skid pad on the bathroom/restroom floors.” And the lowest score was "the using situation of the non-recovery movement equipments”. After living into this organization,the mean value of the patient health changes, were situated between 4.23 to 4.27,most of the residents became happily than before (59.1%). In concerning of the individual data how to affect the each item of degree of satisfaction, in the sex item regards to satisfied degree of the “indoor air quality” in the hardware part and the "selectivity of the daily menu " in the software part both had the significant of differences (p<0.05 and p<0.01). Furthermore we find the different marital status of the samples that existed different mood situations which had the significant degree of satisfaction (p<0.0). We hope these findings may promote the service quality thinking and to help the residents to obtain their more suitable care and the better living conditions. |
