中文摘要 | 本實驗採用12位口腔黏膜下纖維化(OSF)患者之組織切片,依據組織病理學診斷分為早期與中期口腔黏膜下纖維化各6例,再加上6例正常健康之口腔黏膜組織(NM)以及4例分化良好(well-differentiated)之頰黏膜口腔癌(OSCC)組織標本,用來當作實驗對照組。將這些黏膜組織以七種不同之lectins (ConA、PNA、UEA-1、WGA、DBA、SBA、RCA)分別加以染色,並比較它們上皮細胞醣類分佈及其分化關係。結果顯示不論OSF程度為何,七種不同之lectin在結合表現上無明顯之差異。Lectins (UEA-1,WGA,RCA)的結合表現在三種不同組織中(即NM,OSF,OSCC)均呈現類似的強力結合(binding),並且此現象以棘狀層(spinous layer)為主,但各組織與這三種lectins的結合強度表現並沒有差異。DBA則在這三種不同組織之結合表現均呈現極弱現象,且彼此之差異不大,不具指標意義。SBA則在正常黏膜組織及OSF組織之結合較弱,而與OSCC組織的結合則有較其他二組織稍強之表現,可作為病變組織間細胞分化轉變之參考指標。Con A在正常黏膜組織之結合較強,在OSF與OSCC呈現較弱至中度之結合染色,也可作為由正常組織轉變為病變組織之參考指標。PNA則在正常黏膜組織(NM)及OSF組織呈現較弱結合,在SCC組織則結合最強,與SBA類似。這些結果顯示Con A,PNA,SBA三種lectins在未來可以作為偵測不同程度之正常或異常組織細胞分化之指標。為達到更精確的臨床偵測指標,還需做進一步的研究,以期能發揮臨床上潛在的價值。 |
英文摘要 | The purpose of this study was to investigate the expression of lectin binding histochemically in oral submucous fibrosis. Twelve cases of clinically diagnosed oral submucous fibrosis (OSF) with evidence of histopathological proof were included in this study. They were further divided histopathologically into 2 groups, one for early stage (6 cases) and the other for moderate stage (6 cases). Four cases of confirmed well differentiated squamous cell carcinoma (OSCC) of buccal mucosa were also included for the study. Normal mucosal (NM) tissues from the oral cavity were obtained from 6 subjects without OSF to serve as controls. Lectins have been utilized as tools to asses the cell surface glycoconjugate changes in these tissues. The lectins used were Con A, PNA, UEA-1, WGA, DBA, SBA, RCA. Most of the biotinylated lectins showed binding of varying intensity and distribution to the different layers of the mucosal epithelium. The results revealed there was not much difference in all 7 lectins binding to OSF in terms of different stages (Table1). UEA-1, WGA, RCA showed similarly positive binding involving spinous layer primarily in OSF as well as in NM and OSCC tissues. DBA showed most negative or very weak binding in all tissues studied. SBA showed negative to weak binding in OSF and similar to NM tissues, but the binding was increased (moderate) in OSCC tissues. This lectin may be used as a cytochemical probe in differentiating malignancy from OSF. Con A showed strong binding in normal tissues but the binding was weak in OSF and OSCC tissues. This lectin may be used as a probe in differentiating normal from diseased tissues. PNA showed strong binding in OSCC tissues and the binding was decreased in OSF & NM tissues. This binding pattern was similar to SBA studied. In conclusion, SBA, Con A, PNA may be utilized as probes to determine the changes of the disease status e.g. OSF, OSCC from normal mucosal tissues of the oral cavity. |
