中文摘要 | 為了探討口腔黏膜下纖維化症(OSF) 發生過程中蘭格罕氏細胞(LC)數 目之改變,我們利用免疫組織化學染色法,以抗GDla和HLADR抗體偵測15例OSF 和15例正常口腔黏膜標本冷凍切片中口腔上皮之LC密度。研究結果發現OSF口腔 上皮中之LC形狀和大小差異頗大,且不均勻地散佈於口腔上皮之棘狀層甚至表 層中。而正常口腔黏膜中之LC主要分佈於上皮之基底和基底旁層且有聚集於結 締組織乳頭頂端上皮中之傾向。OSF口腔上皮中LC密度明顯低於正常口腔黏膜上 皮中LC密度(p<0.001),此意謂OSF口腔上皮中局部免疫監視能力可能降低。我們 認為OSF口腔上皮中LC密度之降低可能和檳榔嚼塊中成分對LC之直接細胞和基 因毒性有關;另外因口腔黏膜上皮下方結締組織纖維化,血流量逐漸減少,而導 致對口腔黏膜上皮中之LC營養供給減少,也可能縮短LC之壽命;而血流量減少, 從血液中移行至口腔黏膜上皮中之LC數目也會減少。總之OSF口腔上皮中之LC 密度降低,可能和OSF上皮容易惡性轉變成口腔鱗狀細胞癌有關。 |
英文摘要 | To evaluate the change in the number of Langerhans cells (LCs) during the development of oral submucous fibrosis (OSF), frozen tissue sections of 15 samples with OSFand 15 samples of normal oral mucosa were immunostained with antibodies againstCDIa and HLA DR to measure the LC density in the oral epithelium. LCs in OSF epithelium demonstrated a large variation of shape and size and were distributed randomly inthe spinous layer and even in the superficial layer of the epithelium. However, in normaloral epithelium, LCs were found mainly in the basal and parabasal layers and showed atendency to cluster in the epithelium around the tips of connective tissue papillae. TheLC density in OSF epithelium was significantly lower than in normal oral epithelium (p<0.001), indicating that local immune surveillance in OSF epithelium may be decreased.We suggest that the reduction of LC density in OSF epithelium may result from thedirect cytotoxic and genotoxic effects of betel quid ingredients, as well as diminishednutritional supply to LCs and a decrease in the recruitment of LCs from the circulationas a result of marked fibrosis and a progressive loss of vascularity in the subepithelialconnective tissue. The reduction of LC density in OSF epithelium may be one of thefactors in increased risk of malignant transformation of OSF into squamous cell carcinoma. |
