中文摘要 | 動脈硬化(atherosclerosis)的過程是一種發炎反應。急性冠心症(acute coronary syndrome)包括不穩定心絞痛(unstable angina)及急性心肌梗塞(acute myocardial infarction),是冠狀動脈硬化的急性併發症。急性冠心症主要是冠狀動脈血管內的粥狀動脈硬化症(atherosclerotic plaque)破裂所引發的一連串急性發炎反應(inflammation)和血栓形成(thrombosis)所致。至於冠狀動脈粥狀動脈硬化斑之所以會破裂,也和發炎反應有關。因此發炎反應無論在動脈硬化形成過程或其導致的急性併發症中都扮演著重要角色。偵測血中指標配合遺傳分析將來應有助於對病人的風險評估及治療。 急性心肌梗塞也會引起發炎反應,包括局部的心肌變化和全身性發炎反應(systemic inflammatory response syndrome)。急性心肌梗塞引發的全身性發炎反應不但在急性心因性休克(acute cardiogenic shock)和急性心因性肺水腫(acute cardiogenic pulmonary edema)的致病機轉上可能扮演重要角色,也可能影響預後。及早以冠狀動脈介入治療(percutaneous coronary interventions)打通堵塞的血管在治療這類病人非常重要。未來對發炎反應的研究可能有助於對致病機轉的瞭解和開發新的治療方式。 |
英文摘要 | Inflammation plays a central role in the genesis of atherosclerosis and its complications. From the initial recruitment of circulating inflammatory cells to the diseased vascular endothelium, to eventual rupture of the vulnerable atherosclerotic plaque, inflammatory mechanisms contribute decisively to all phases of atherosclerosis and its complications. The combination of the serum markers and hereditary pre-disposition revealed by genetic analysis should sharpen our ability to make a prescription for management of risks in individuals in a rational manner. New evidence also suggests that local and systemic inflammatory responses occur in the setting of acute coronary syndrome, including acute myocardial infarction. Inflammation and immune activation may play an important role not only in the development of cardiogenic shock or pulmonary edema complicating acute myocardial infarction but also in outcome after the episodes. Immediate stabilization followed promptly by early revascularization is recommended in these patients. Identifying the triggers for inflammation and unraveling the details of inflammatory pathways way eventually furnish new insights and therapies. |
