中文摘要 | 機械通氣提供呼吸衰竭患者呼吸方面的輔助,近來使用機械通氣得數量有增加的趨勢,是重症加護單位的重要治療模式之一。使用機械通氣時,須熱知它對身體各器官所產生的影響及合併症,它括心臟胸腔血管方面的心輸出量減少、肺臟的損傷(氣壓傷、容積傷、不張傷、生物傷、自發性吐氣末正壓)、腦部血液回流減少及顱內壓力增高、腎臟功能異常及腸胃系統方面的腐蝕性食道炎、壓力有關的黏膜損傷、腹瀉等問題。我們應對於患者進行詳盡的身體評估,選擇正確的處理方式,再加上各種呼吸力學的監測方法,給予患者一個安全的醫療處置。 |
英文摘要 | Mechanical ventilation is supposed to help patients with respiratory failure. The use of mechanical ventilation has been increasing in recent years and becomes a major therapeutic modality in intensive care units. It is essential for physician to know the effects complications of mechanical ventilation on different vital organs and prevent related complications. The complications include decreased cardiac output; lung injury (barotraumas, volutrauma, atelectrauma, biotrauma and auto-PEEP); decreased cerebral perfusion, increased intracranial pressure, change in renal function, erosive esophagitis, systemic inflammatory response syndrome and diarrhea. Before initiation of mechanical ventilation, physicians or medical personnel have to perform physical examination to the patient meticulously, and then choose approach accurately. To apply respiratory monitoring during mechanical ventilation is also very important to keep patients away from the above-mentioned complications. |
