| 英文摘要 | Cemento-ossifying fibroma (COF) is a rare, well-demarcated, benign fibro-osseous tumor composed of lamellar bone, fibrous tissue, and cementum. COF occurs in the sinonasal tract, maxilla, calvarium, and mandible. Deterioration of visual acuity associated with the involvement of the paranasal sinuses has not been widely reported. We here report such a case. A 20-year-old male experienced progressive deterioration of visual acuity and proptosis over 8 years. A heterogeneous mass was revealed by computer tomography, causing destruction at the left frontal, ethmoid, sphenoid, and anterior cranial fossae, with optic canal and orbital involvement. Magnetic resonance imaging demonstrated a hypointense cystic lesion in the left spheno-ethmoid sinus region with encasement of the left optic canal. The fibro-osseous lesion of the left spheno-ethmoid sinus was associated with compressive optic neuropathy. With partial ethmoidectomy, a hard cotton-like tumor surface was exposed, encasing a yellow-whitish fluid. Pathologically, psammoma body-like spherules with foci of trabecular bony tissue and associated fibrous stroma were prominent. These findings were compatible with COF. Radical resection will be necessary to control this histopathologically benign, yet behaviorally aggressive, lesion. |
