| 中文摘要 | 登革病毒為RNA病毒,屬於黃病毒科,主要由節肢動物──蚊子叮咬人體後而感染。登革熱的流行在熱帶或亞熱帶地區都是重要的公共衛生問題。台灣之地理位置屬於亞熱帶,亦是登革熱流行的地區之一。登革病毒可依血清型分為四型,第一型至第四型 (DEN-1 to DEN-4)。四型的登革病毒感染後之臨床表現均相 似,其疾病的程度可由無徵兆到輕微登革熱再至嚴重的登革出血熱及登革休克症候群。 有關於登革出血熱及登革休克症候群的致病機制,有兩個主要的學說。免疫增強學說認為疾病的嚴重度與病毒於巨噬細胞中繁殖的程度相關。試管外實驗顯示,初次感染後產生的非中和性之交互作用抗體,可促進登革病毒在巨噬細胞及單核球中繁殖;另一學說為病毒學說,認為由於登革病毒之基因變異,可形成較具有致病性的登革病毒。而登革病毒感染人體後,會引起體內對抗病毒蛋白質之抗體生成,而抗體在疾病不同階段所出現的型式對於疾病的嚴重度之意義,目前並非十分清楚。 本實驗著重於探討登革病毒感染後,體內抗體反應的型式 (pattern) 及其與疾病嚴重程度的關係。本實驗的第一個目的是探討登革病毒感染後,體內抗體表現之型式,我們利用西方墨點法 (Western blot) 偵測,其結果顯示,包括套膜蛋白 (E protein) 及數種非結構蛋白 (NS protein) 皆可被偵測出。第二個目的是探討 不同抗體表現之型式是否與疾病嚴重度有關。而結果亦顯示,在我們偵測2001年台灣高雄地區二十六位的登革熱及登革出血熱病人血清中,抗體出現之型式與疾病之嚴重度並無相關。 |
| 英文摘要 | Dengue virus is a positive, single stranded RNA virus and belongs to the family Flaviviridae. It is transmitted by mosquitoes. Epidemics of the four dengue serotypes (DEN-l to DEN-4) continue to be a major public health problem in tropical and subtropical regions including Taiwan. The clinical manifestations of dengue range from a mild febrile dengue fever (DF) to a severe and potential life-threatening dengue hemorrhage fever/ dengue shock syndrome (DHF /DSS). There are two major hypotheses which have been proposed regarding the pathogenesis of DHF /DSS. The immune enhancement hypothesis proposed that the severity of disease is related to the magnitude of viral replication in monocytes, which has been shown to be enhanced by the presence of cross-reactive antibodies in vitro. The viral hypothesis proposed that the severity of disease is related to the strains of viruses. There was evidence indicating that the antibody responses to different dengue proteins vary between different individuals. Whether different patterns of antibody response correlate with different disease severity remains to be investigated. This study elucidates the pattern of antibody response after dengue virus infection and it also related to the severity of disease. The data show that E and several NS proteins could be detected including DF and DHF patients. We used Western blot analysis in patients with different disease severity. These results revealed the pattern of antibody response were not correlation with the disease severity. |
