中文摘要 | 使用呼吸器傳統上多數使用容積控制型通氣模式(Volume control ventilation, VCV),優點為可提供固定潮氣容積,但會忽略因高氣道阻力或不良的肺彈性,而併發有肺部氣壓傷之虞。此外,吸氣流量若未隨病人需求調整,亦可能出現病人與呼吸器間之不協調而導致呼吸功增加。另一類選擇為壓力控制型通氣模式(Pressure control ventilation, PCV),藉由控制吸氣壓力和吸氣時間,因而避免肺容積及壓力傷害的機會,並且使肺部持續保持充氣,有助於吸入氣體之均勻分佈。VCV和PCV模式,各有其優缺點及使用機制。近年來發展出所謂的雙控模式(Dual-control modes),乃藉由結合容積與壓力之雙重特點,以期達到控制壓力之餘又能保証吸氣容積的新穎型通氣方式。目前市面上已推出各種不同的機型包括有:(1)8400Sti的容積保証壓力支持型(Volume-assured pressure support, VAPS)、(2)Bear 1000的壓力增強型(Pressure augmentation, PA)、(3)Siemens 300呼吸器的容積支持型(Volume support, VS)、(4)Siemens 300的壓力調節容積控制型(Pressure-regulated volume control, PRVC)、(5)Drager evita 4呼吸器之自動流量(Auto-Flow)及(6)Hamilton galileo呼吸器的調適輔助通氣型(Adaptive support ventilation, ASV)。機型及原理各不同,本文將一一作介紹。 |
英文摘要 | The most conventional application of mechanical ventilation is volume control ventilation (VCV). Besides providing constant tidal volume, VCV can bring complications of lung volume or pressure injury while disregarding for high airway resistance or poor lung compliance. However, dyssynchrony between patient and ventilator can be developed and results in aggravated work of breathing, when the flow amount cannot fit patient's demand. Pressure control ventilation (PCV) is another option in clinical practice. Avoidance from lung injury by controlling the inspiratory pressure level and time, PCV is also available to equilibrating gas distribution by continual alveolar insufflation. There are many discriminations between VCV and PCV in applications and restrictions. In contemporary development, it raises novel dual-control modes which are integrating both characteristics in order to control pressure level and guarantee inspiratory tidal volume in the meantime. Nowadays, there are many different kinds of machine models on the market, such as (1) Volume-assured pressure support, VAPS, in 8400 Sti, (2) Pressure augmentation, PA, in Bear-1000, (3) Volume support, VS, and (4) Pressure-regulated volume control, PRVC, in Siemens 300, (5) Auto-flow in Drager Evita 4, and (6) Adaptive support ventilation, ASV, in Hamilton Galileo. This article will introduce and discuss those new modes individually. |
