中文摘要 | 本研究目的在比較現今臨床常用的神經外科加護照顧監測方法和腦部微量透析監測方法的效率和關聯性。作者提出三位急性腦傷病人,其中包含嚴重頭部外傷,和腦血管病變,需要神經外科加護照顧,我們合併使用神經生理監測方法和腦部微量透析監測方法來監測病患。因為治療顱内壓昇高,改善腦部血流,和腦部氧氣代謝,對於此類病人是非常重要,且能改善病人預後。從我們結果得知,腦部微量透析方法優於目前所使用的監測方法,包括顱内壓監測器,昏迷指數,腦血流擴散壓力,組織氧濃度,頸部脈氧飽和度等傳統的監測方式,因為微量透析能夠連續直接提供我們大腦皮質神經元細胞外液神經化學組成成分變化的資料,能更早監測繼發性腦傷發生。但是神經外科加護照顧監測並不能單靠一種顧監測方法來完成,而是需靠各種不同的監測方式,且小心謹慎研判各種監測結果來做最好的治療。如此對需要神經外科加護照顧的病人才能有所幫助。 |
英文摘要 | Objective: This study compares the effects of modern neurointensive care monitors in clinical use and the authors try to detect new correlation between traditional parameters and cerebral microdialysis monitoring. Methods: In 3 patients, with acute brain injuries (severe head trauma and cerebrovascular disease) requiring nerurointensive care, neurophysiological monitoring and/or cerebral microdialysis monitoring was performed. This monitoring includes intracranial pressure (ICP), cerebral blood flow (CBF) and cerebral oxygen metabolism, which are vital and may improve the outcome for these patients. Results: Cerebral microdialysis is better than other monitoring methods, including ICP monitoring, GCS, cerebral perfusion pressure, and jugular venous oxygen saturation monitoring as it supplies continuous neurochemical information directly from the extracellular space of the cortex and detects delayed hemorrhage earlier. Conclusion: From the experimental results, it is evident that modern neurointensive care monitoring should be multi-modality monitoring. No single monitoring method should be relied upon alone, and the results of which must be carefully analyzed and understood. Therefore, the neurosurgical patients benefit from these modern neurointensive care monitors. |
