中文摘要 | 背景:慢性扁桃炎是由於扁桃體反復感染所造成,好發於4到8歲之間的幼童。扁 桃體在幼兒期的免疫作用扮演較重要的角色,其其全部切除,有可能會影響到人體的免疫功 能。傳統扁桃切除術(tonsillectomy)除了影響免疫系統的疑慮外,還有術後疼痛與出血的 缺點,使病童家長較無法接受幼童期就破壞了口腔完整性及對幼兒手術安全性產生畏懼感, 而拒絕扁桃切除術。我們以二氧化碳雷射進行扁桃切割術移除聚集於扁桃體隱窩的發炎反應 ,可減少術中、後出血及術後疼痛,且術後仍保留扁桃體組織結構,家長滿意度甚高,可為 幼童慢性扁桃炎提供另一種新的治療方式。 方法:自民國85年12月起,本科對12例年齡4到8歲有慢性扁桃炎併上呼吸道阻塞之幼童予以 診療並列入研究,用問卷調查方式查明症狀發作情形後,以二氧化碳雷射進行扁桃切割術( tonsillotomy),將扁桃體部份切除,術後於門診追蹤7個月至1年4個月,評估反復喉嚨痛、 喉異物感及呼吸道阻塞3種症狀的改善情形。 結果:12個病例中,除1例術後輕度發燒外,術後反復喉嚨痛、喉異物感及呼吸道阻塞症狀 皆獲得改善,且無術後合併症及復發現象。 結論:二氧化碳雷射扁桃切割術可減少術後疼痛及出血,並保留扁桃體組織結構及其相關免 疫機能,經半年以上追蹤未有復發且效果良好。所以我們認為二氧化碳雷射扁桃切割術是一 種安全有效的手術方法。 |
英文摘要 | Background: Chronic tonsillitis is the result of recurrent inflammation of the palatine tonsils and the peak incidence is between 4 and 8 years of age. Palatine tonsillectomy is the treatment of choice. However, this surgery can result in some post-operative complications, such as severe pain and bleeding. Moreover, the palatine tonsils are believed to play a role in mediating immunities, hence an early and total removal of these lymphoid structures might impede the full developmental potential of immunity in children. Dismantling of the oral structures also prohibited the parents' consent. We hypothesized that CO�眩onsillotomy, in which a minimal amount of tonsil tissue is retained, could possibly bring a reconciliation between symptomatic relieves and better immune development. Methods: From December 1996 to December 1997, pediatric patients between the age of 4 and 8 diagnosed as having chronic tonsillitis with symptoms of upper airway obstruction underwwent CO�疹aser tonsillotomy and followed regularly after the operation. Pre- and postoperative questionnaires were used to investigate the degrees of symptomatic relief. Results: We collected 12 subjects and followed up for 7 to 16 months. Except for one patient who developed mild fever after tonsilotomy, all subjects showed no postoperative complications and experienced decreased frequency of recurrent sore throat, improved lump throat sensation and air way obstruction. The initial results were promising. A decrease in complication rates and partial preservation of tonsils has received a good parental satisfaction. Conclusion: The results of our pilot study indicated that CO�疹aser tonsilotomy could effectively reduce postoperative complications, relieve tonsillitis symp- toms and possibly retain better immunological development. |
