中文摘要 | 氣喘為一過敏疾病會有嗜酸性白血球含量增加,氣管發炎細胞浸潤、氣管細胞膜不穩定,釋放發炎介質之一LTC4,引起氣管平滑肌強烈收縮,造成氣道阻力提高,嚴重影響肺功能。固本一號為治療氣喘之中藥處方,其組成:黃耆0.5g、黨參0.5g、茯苓0.5g、橘皮0.25g、白朮0.5g、半夏0.25g、甘草0.25g、紫河車0.25g、補骨脂0.3g。 本研究目的在於探討:固本一號是否俱有增強發作期氣喘天竺鼠之肺功能與降低氣喘天竺鼠在激發過敏反應之敏感度,增加潮氣量及減少支氣管阻力的效能,其改善之作用機制是否經由穩定肺泡薄璧組織結構、改善發炎細胞在支氣管黏膜下層的浸潤情形、降低嗜酸性白血球含量、增加巨噬細胞與OVA特異性Ig G數量,使發炎氣管之細胞膜更加穩定。最後以中醫氣喘治療方劑固本一號之組成藥物,逐一地探討各個藥物個別對氣喘天竺鼠之肺功能的影響,其療效是否與固本一號相同。 實驗程序是先將實驗動物天竺鼠誘發成氣喘,以OVA及methacholine噴霧激發試驗下表現出嚴重氣喘症狀,在確認氣喘後餵食固本一號每日兩次每公斤0.2公克,連續二週後再以OVA激發後測試肺功能及肺敏感度,用ELISA實驗法測試天竺鼠之OVA特異性Ig G,最後取實驗動物肺臟進進行病理切片及檢測發炎細胞數量。 實驗結果:I、天竺鼠在OVA誘導刺激後之肺功能與敏感度,固本一號可增加氣喘鼠之潮氣量,65%(早期第一分鐘)、40%(晚期第六小時)、13%(第二十四小時);降低支氣管阻力162%(第五分鐘)、112%(第六小時)、31%(第二十四小時)。在1.25mg/ml最高methacholine濃度刺激後之肺敏感度,固本一號組之潮氣量比氣喘組多出15.8%;支氣管阻力則比氣喘組減少95%。Ⅱ、天竺鼠肺組織在氣喘發生後之變化,固本一號組其肺泡細胞較正常肺臟略有增生,小支氣管周圍無明顯炎症細胞浸潤,且小支氣管內纖毛完整,內皮細胞脫落少,結構上完整性高,無肺泡空間阻塞情形出現。III、天竺鼠血清中OVA專一性Ig G及之變化及發炎細胞計量,固本一號組之嗜酸性球較少、巨噬細胞增加;固本一號組在OVA激發後Ig G上升四倍之多,補骨脂組上升約60%。 結論:天竺鼠氣喘現象非常明顯,此動物模式可應用於藥物藥效的研究。本實驗中藥氣喘治療方劑固本一號,確實可改善天竺鼠氣喘動物之肺功能與肺敏感度。固本一號可改善小支氣管上皮剝落程度、增加微纖毛結構完整性、減少嗜酸性球、增加巨噬細胞數量且可促進OVA專一性Ig G之增生,而其詳細的治療機制仍然有待更進一步地探討。中醫氣喘治療方劑固本一號之單一組成補骨脂藥效不及整體處方,其他組成的藥效仍有待進一步探討。 |
英文摘要 | Bronchial asthma (BA) is an allergic disease, increased infiltration of eosinophils and increased intracellular spaces of bronchial epithelium, released of LT C4 to induced bronchial obstruction. Chinese herbal prescription, Ko-Ben No.1 (K.B No.1 ), is the extract of Huangzi 0.5g, Dangshen 0.5g, Fuling 0.5, Jupi 0.25g, Baizhu 0.5g, Banxia 0.25g, Zihache 0.25g Gangcao 0.25g Buguzhi 0.3g。 In this study we observed the efficacy of K.B. No.1 in the treatment of BA guinea pig. We detect their airway hyperreactivities and compare the concentration of IgG, eosinophils, macrophage and the lung histopathological examination. Guinea pigs body weight 400g was divided into 2 groups: the negative control group was treated without any vaccination. The others were aerosolized-OVA sensitized then group into 3 categories :K.B. No.1 group(KB), Buguhzi group (BG) and placebo (BA) group. Each group were given for 2weeks twice a day (0.2/Kg). BA group presented severe early and late-phase airway hyperresponsiveness after OVA and methacholine challenge; KB group showed the least airway hyperreactivities that increased 65% tidal volume in 1st min, 40% in 6th hr; decreased 162% bronchial resistance in 5th min, 112% in 6th hr. The lung histopathological examination of the 3 groups, all of them except the KB group showed severe proliferation of alveolar wall and became firmly fibrotic, and narrowing of the alveolat spaces were noted. The OVA specific IgG was increased four times in KB group and 60% in BG groups. The phagocytotic activity of macrophage was increased and the eosinophils was decreased in KB group. These results indicate that OVA-induced guinea pig model of asthma is more similar to asthmatics. It's a good model of asthma for the study of the therapy. K.B. No.1 could prevent OVA and methcholine induced airway hyperreactivities. The OVA specific IgG was increased and the eosinophils was decreased after been given K.B. No.1. The prescription K.B. No.1 is more effective than Buguhzi. |
