中文摘要 | 磁振造影是目前臨床診斷之重要工具之一,特別是在檢查中樞神經系統以及軟組織病灶時,磁振造影是不可或缺的診斷利器。然而在操作磁振造影儀器之前,工作人貝必須考量其安全性以避免醫源性之併發症。含鐵而具有磁性的物質在電磁場下會產生電流(eddy current)而放熱的現象,很可能是磁振造影檢查導致灼傷的主要原因。因此,患者身上接觸磁振造影儀器附件或者各種生理監測裝置時,在進行檢查前必須瞭解其是否具有危險性以避免灼傷的發生。由於磁振造影檢查而導致灼傷的個案在台灣未曾被報導過,本文在此提出一患者在磁振造影檢查時因心電圖同步監測而導致胸部全層皮層灼傷之個案報告,提供各位參考並希望能避免此併發症的發生。 |
英文摘要 | Magnetic resonance imaging (MRI) is an important diagnostic tool. MRI is indispensable in diagnosis of the central nervous system and soft tissue lesions. Prior to a clinical MRI examination, various safety considerations should be emphasized to avoid the occurrence of iatrogenic complications. Burn injury resulted from MRI is a rare complication and which has never been reported in Taiwan. Although the cause of the MRI induced burn remains theoretical, the electrical current (eddy current) induced by the magnetic and radiofrequency fields may be the major cause of the injury. Therefore, the MRI accessories or the physiologic monitoring devices should be thorough tested and determined to be safe for patient during MRI procedure. We report a case that a patient under general anesthesia suffered from full-thickness burns due to the use of electrocardiographic (ECG) monitoring leads during an MRI examination. |
