中文摘要 | 摘要:為了探討抗血小板製劑應用於血液透析之效果,本實驗以5隻體重23-33公斤 健康成犬為研究對象,進行血液透析試驗,實驗共分 3 組, 分別為肝素組 (H 組,肝素劑 量為初劑量 40IU/kg,維持劑量 30IU/kg/hr),肝素 + 低劑量 Ticlopidine 組 (H+T 組, TiCIopidine 劑量為 50mg/kg/day), 肝素 + 高劑量 Ticlopidine 組 (H+2T 紐, Hc10pidine 劑量為 I00mg/kg/day)。 HC10pidine 於透析前 3 天開始投予,每天一次,投 藥前並監測實驗犬之出血時間、 活化部分成栓質時間 (APTT) 、 血球數、 丙胺酸轉氨脢 (ALT)、血中尿素氮 (BUN) 及肌酸酐 (Creatinine) 等值之變化。 每次透析時間為 3 小時 ,評估方法包括透析過程中血球數及透析結束後體外循環通路及透析器中之殘留固體血液重 量等。藥物副作用之檢測結果顯示,Ticlopidine 並未造成血球數、血液生化及凝血試驗等 方面之明顯變化。透析過程中所檢測之結果則顯示,H+T 組與 H+2T 組之白血球降低程度顯 著地較 H 組輕微,而 H+T 組與 H+2T 組間則無明顯差異; 此外, H+2T 組之血小板降低程 度亦明顯小於其他兩組。 體外固體血液重量之比較顯示, H+2T 組之凝血量最少, 其次為 H+T 組,最重者為 H 組,三組間之差異皆達統計顯著水準。 根據以上之結果,顯示血液透 析時以肝素併用適當高劑量之 Ticlopidine 抗凝, 將可有效地防止血小板及白血球數的減 少,並防範過多凝血塊之產生。 |
英文摘要 | The efficacies of low-dose heparin and low-dose heparin with ticlopidine as anticoagulants during hemodialysis in dogs were compared and evaluated. Five healthy dogs of body weight ranging 23~33 kg were hemodialyzed. Group I was hemodialyzed with heparin as the sole anticoagulant which was administered at a dose of 40 IU/kg (initial dose ) and 30 IU/kg/hr (maintenance dose ). Groups II and III were hemodialyzed with heparin at the same dose and together with two regimens of ticlopidine, 50 mg/kg/day and 100 mg/kg/day respectively. Ticlopidine was administered orally once a day at the doses mentioned above 72 hours prior to hemodialysis, and the evaluation of ALT ( alanine aminotransferase ), BUN (blood urea nitrogen ), creatinine, blood cell count were carried out before each administration. The period of each hemodialysis was 3 hours. Parameters monitored during hemodialysis included blood cell count and residual blood weight. No significant differences of those evaluated parameters mentioned above were found among dogs which were pretreated with ticlopidine. A significantly lower degree of hemodialysis-associated leukocytopenia was found in both groups II and III than that of group Ⅰ. However, no significant differences were observed between the groups II and III. In addition, dose-dependent reduction of residual blood weight was observed in the groups II and III. A much less degree of hemodialysisassociated thrombocytopenia was observed in group III as compared to group 1. According to the salutary effects demonstrated above, the combination of heparin and higher ticlopidine regimen may be more clinically beneficial. |
