中文摘要 | 本文由社區心理衛生三級預防的觀點進入學校心理衛生之探討,嘗試建立新的校園心理衛生工作模式及各項工作要項,由學校心理衛生工作中,可以見到助人工作之重疊與互補關係,藉著學校心理健康推展工作了解教育行政上對於學校心理健康之主軸及推展項目;在精神醫療團隊參與學校心理衛生工作中,最直接的應是兒童青少年專科醫師及其模式之互動關係,而目前精神科專科醫師之養成模式尚不足,甚至精神醫療與學校心理衛生研究亦缺乏全面之合作關係;反觀從事學校心理衛生工作之主體輔導工作亦是在起步階段,專業證照之限制及輔導工作之現代化與團隊合作等均是仍待努力之目標;文中進一步以高雄醫學大學進行7年之例,檢討工作上必須再推展之方向。 |
英文摘要 | This article discusses mental health issues affecting school students and their primary, secondary & tertiary preventions. The differences between the school-based and community based mental health care are explored and methods for cooperation are discussed. The Mental Health Act in Taiwan (1990) stipulates that psychiatrists should help to promote the school mental health as well as promoting mental health in the community. During school, students learn interpersonal and social behaviors in addition to general education. The school environment is designed to provide assistance to students with mental health related problems through the school counseling center. The goverment set up programs to improve the effectiveness of school counseling systems. Current psychiatric training programs do not adequately prepare psychiatrists for the school mental health environment . Also the tasks of education, guidance, counseling and psychotherapy are somewhat overlapped. School mental health policy is formulated by many specialists that should cooperate with each other using client centered methods. Communication between psychiatrists and school counsellors is important to mental illness prevention in schools. The authors also present a model of comprehensive counselling center network used for 7 years in Kaohsiung Medical University. The network provided general guidance by tutors and high risk survey by counselors as its primary function in preventing student mental illness. As a secondary prevention function the center offered crisis intervention and acute psychiatric hospitalization. Psychiatric referral or aftercare was also provided by counsellors to help students' with readjustment. This has been in operation for years and focuses on maintaining communication among teachers, tutors, counsellors, psychiatrists and administrators. The results of this investigation suggest that psychiatric residents training should include individual counseling, mental health education, and that the concept of team approaches should be emphasized. New model of school mental health should emphasize cooperation between psychiatrists and school counsellors. |
