中文摘要 | 現代醫學對過敏性鼻炎有許多的治療方式,而傳統醫學對肺氣虛型過敏性鼻炎患者以玉屏風散治療。歷代玉屏風散劑量比例不同共四組,為了以現代醫學科技探討玉屏風散對過敏性鼻炎施治的作用機轉,我們首先研究此方劑對於抗過敏作用及調節免疫因子之影響,並研究那一組對於細胞免疫反應正向作用最明顯,以期為臨床應用提供科學數據,且有助於有效成份之分析。因此針對歷代玉屏風散組成相同(黃耆:白朮:防風)劑量比例不同四組,G組=1:1:1,H組=1:2:1,I組=6:4:3,J組=3:1:1,進行單核球分泌間白素-4(Interleukin-4, IL-4)與中性白血球分泌組織胺(histamine)影響之研究。結果顯示:玉屏風散可以有效降低histamine及IL-4的濃度(p<0.05),進一步分析,結果顯示:以抑制histamine及IL-4而言,玉屏風散G組最佳,J組最不明顯(p<0.05)。若以濃度而言:玉屏風散G組於100ug/mi具最佳作用:且得知玉屏風散組成中,防風含量比例愈高與黃耆含量比例愈低之組合,可由本研究體外試驗得較佳之指標:因此我們認為玉屏風散於治療過敏性鼻炎可能經由抑制histamine和IL-4的釋放;且主要由其組成中防風扮演重要的角色,同時亦推測於臨床上治療過敏性鼻炎,若黃耆、白朮、防風之劑量適當之調整,應更具療效。期望本實驗能為整合中西醫學建立一可行的研究模式。 |
英文摘要 | Modern medicine has many methods for treating allergic rhinitis. In traditional Chinese medicine, however, lung-deficiency type allergic rhinitis patients are treated with Yu Ping Feng San. Throughout the years, four different prescriptions of Yu Ping Feng San have been developed, each containing a different proportion of the same ingredients. To investigate the mechanisms involved in the treatment of allergic rhinitis with Yu Ping Feng San using modern medical techniques, we first studied the effect Yu Ping Feng San had on the anti-allergy and regulatory factors of the immune system. We also attempted to determine which formula had the greatest positive effect on cellular immunity, to provide a scientific basis for clinical applications and further analysis of the active ingredients. Four different prescriptions of Yu Ping Feng San, each containing the same ingredients (Huangqi, Baizhu and Fangfeng) in different proportions, were labeled: G (1:1:1), H(1:2:1), I(6:4:3), and J(3:1:1). Experiments were then conducted to investigate the effect these four prescriptions had on interleukin-4 (IL-4), secreted by monocytes, and histamine, secreted by neutrophils. Results showed that Yu Ping Feng San effectively reduced the concentration of histamine and IL-4(p<0.05). Further analysis revealed that, in terms of inhibiting histamine and IL-4, sample G produced the greatest effect, especially at a concentration of 100 μg/ml, while sample J produced the least effect (p<0.05). We also found that prescriptions containing higher proportions of Fangfeng and lower proportions of Huangqi gave better indications in the in vitro experiments conducted in this study. Therefore, we believe that Yu Ping Feng San can alleviate conditions of allergic rhinitis by inhibiting the release of histamine and IL-4, with one of its ingredients, Fangfeng, playing a major role. We also propose that in the clinical treatment of allergic rhinitis, appropriate adjustments in the dosages of Huangqi, Baizhu and Fangfeng will produce greater therapeutic effects. We hope this investigation will provide a useful research model for the integration of Chinese and western approaches to medicine. |
