中文摘要 | 以電腦協助分析的角膜地型圖是評估角膜表面特徵及局部病變最好的方法。本文 目的在評估即將接受雷射屈光手術患者的角膜地型圖,並將其分類,決定各種型態的頻率以 及與其它因子的關係。 我們使用EyeSys公司的角膜分析系統(Corneal Analysis System 2000)來分析並將 這些角膜地型圖分類,總計有83個病人166隻眼睛被包括在本實驗。我們採用彩色圖最中 間的顏色—綠色來做型態的分類而將這些角膜地型圖分成六大類。 各種型態之頻率分別為:圓形佔21.7%,橢圓形佔22.9%,對稱性蝴蝶結領帶型佔16.9%, 不對稱蝴蝶結領帶佔9.6%,腎臟型佔10.8%,不規則型佔18.1%。在各種不同型態間,角膜 彎度性散光及屈光性散光這兩項有差異存在。而這差異主要是由於蝴蝶結領帶型和其它型態 不同所導致,而對稱性和不對稱性蝴蝶結領帶兩型間則沒有明顯差異。 以電腦協助分析的角膜地型圖能快速而正確的提供角膜的資訊,即使使用之器械不同可 能造成差異,但以彩色圖及其中間值綠色來做角膜地型圖型態的分類仍是適當而可行的。 |
英文摘要 | Computer-assisted corneal topography is a sensitive tool for assessing the corneal surface. The purpose of this study is to evaluate the corneal topography of photorefractive keratectomy candidates. We try to classify the patterns of corneal topography, then we also seek to determine the frequency of patterns and parameters of corneal topography. Corneal Analysis System 2000 (EyeSys Technologies) was used to analyze and classify the corneal topography of 83 subjects receiving prephotorefractive keratectomy screening. The patterns were defined by using the mid-dioptric green color and classified into six pre-defined categories. The pattern frequency was round (21.7%), oval (22.9%), symmetrical bow tie (16.9%), asymmetrical bow tie (9.6%), kidney (10.8%), and irregular (18.1%). Significant differences among patterns were identified in keratometric and refractive astigmatism (p<0.001 and p=0.0013 respectively), and bow tie patterns were significantly different from round patterns (p<0.001), oval patterns (P=0.003), or kidney patterns (p = 0.002). There was no difference between symmetrical and asymmetrical bow tie patterns in keratometric or refractive astigmatism. Computer-assisted corneal topography can provide rapid and accurate information about corneal surface features. The classification of topographic patterns with color-coded map, mid-dioptric green color is adequate, though distribution of patterns may be instrument-dependent. |
