中文摘要 | 本研究選擇一所有病床數近千床、員工人數超過1500人的大型教學醫院,進行環 境及個人噪音測定,評估勞工安全衛生法中噪音恕限量之符合情形及醫護人員與住院病人由 噪音所致的厭煩度問卷調查。主要目的在於探討醫院噪音危害及厭煩度問題,期望本研究是 為建立本土性醫院內部噪音基線值之開始,並依研究結果研提醫院內環境音量建議值。結果 發現: 1.無論24小時(64.9dB±4.2)或10分鐘(64.9dB±6.8),環境音量測值都遠超過美國 環境保護署對醫院的管制標準──白天不超過45dB,夜間不超過35dB。 2.醫院內之急診、開刀房、供應中心、醫事部門及掛號批價處等與病人、家屬接觸的緊 急或行政單位,都有較高的環境音量。 3.各科普通病房護理人員之個人噪音暴露平均音量70.4dB。 4.問卷調查中,無論住院病人(33.8%)或醫護人員(26.5%)皆認為環境中的主要噪音 源來自醫護人員、其他病人或訪客的大聲交談或呼叫;其次為呻吟聲或哭聲;此外由醫療設 備所發出的警報聲或由修繕工程產生的突發或衝擊性噪音都是干擾環境的噪音源。 5.由邏輯斯諦複迴歸分析結果顯示:無論環境音量或個人噪音暴露量,都與醫護人員或 住院病人厭煩度的產生具有統計上顯著的影響。 因此建議:若能控制24小時醫院環境音量於60分貝以下、24小時個人噪音暴露量於70 分貝以下,將可使50%的人不致產生厭煩情緒。由結果得知,有效的執行噪音改善計畫應 從醫院內工作人員的教育指導著手。為建立本土性醫院內的噪音基準值,建議應擴大調查的 對象及範圍,進而訂定一套適合國情的標準,以落實勞工安全衛生法中相關的規定及管理。 |
英文摘要 | The noise has been found to have ill effects on health care workers and patients in the last two decades. However, the regulation on permissible level of noise exposure for health care workers has not been established yet. The aim of this study is to evaluate the compliance of the Occupational Safety and Health Law and propose a indoor noise exposure standard for health care workers in Taiwan. In addition, we tried to find noise-induced annoyance felt by health care workers and patients, and investigate sources of noise exposure in the hospital and control them The noise exposures were assessed by B&K instruments in a large hospital(>1,500 employees and 1,000 beds) in Taichung city. The range of noise exposure of health care workers in the hospital is from 61.4Dba TO 81.4dBA by personal dosimeter and most noise equivalent to sound levels exceed 60 dBA by ambient monitoring. Although the level of noise exposure was lower than the protective criteria of hearing conservation that we have, noise-induced annoyance seemed to exist among health care workers and patients. Therefore, we recommend that a regulation on permissible level of noise exposure that may induce annoyance be set up. |
