中文摘要 | 心臟形態是病理學上之重要表現並與疾病機轉與預後關係密切,本研究之目的是 利用食道超音波影像形成之三維影像資料組以使用曲面曲率分析等之方法來提供局部或整體 的左心室形態分析。研究之步驟為:食道超音波影像的擷取,邊界描繪三維重建與形態分析 參數之計算。產生之形態分析參數有:(1)左心室曲面變化、(2)左心室曲面曲率、(3)左心室 曲面距離、(4)左心室正規化距離、(5)有效半徑、(6)有效半徑差,作為左心室形態分析之 量化分析方法。我們將本形態分析方法套用於數學模型皆有預期之結果,並使用氣球假體進 行使用者間之差異評估,驗證本分析方法之準確性並不會因使用者的不同而影響結果,最後 再將本法應用於臨床影像。這項新的形態分析方法提供了完整而簡易的方法量化左心室整體 與局部之形態描述,不論是二維或三維之心臟超音波影像。我們也提供了正常心臟之形態分 析參數供非正常案例之病理研究參考。 |
英文摘要 | Cardiac shape is an important pathophysiologic adaptation, which has prognostic implications. The aim of this study is to present a novel approach using 3-dimensional (3D) volumetric data set which obtained from the ultrasound images, to perform surface curvature analysis that yields multi-parametric data on regional and global left ventricular (LV) shape. The steps to perform shape analysis consist of 3D acquisition of 2D images, edge extraction, 3D reconstruction, and shape parameter calculation. This yields the following shape parameters: (I) RSC, regional surface change of LV; (2) GSC, global surface curvature of LV; (3) SD, surface distances; (4) ND, normalized distances; (5) ER, effective radius; and (6) ER difference. First, we applied it to six mathematical models to validate the method. The analysis reproduced shape parameters identical to those predicted by mathematical modeling. Then, 3D images obtained (in vitro) of balloons with various shapes were examined. These yielded parameters in concordance with true shapes of the balloons, verified by mathematical modeling. Subsequently, we analyzed left ventricles from humans. |
