| 英文摘要 | In case of herpes zoster affecting the trigeminal nerve, the close relationship between the in volvement of the nasociliary branch and ocular complications is well known. The most striking clinical feature is the presence of a herpetic rash on the tip of the mose, the so-called Hutchinson's sign. This study assessed the reliability of Hutchinson's sign in the prediction of ocular complications. A total of 93 patients hospitalized for herpes zoster of the ophthalmic distribution of cranial nerve V (HZV□) were evaluated. Ocular complications were noted in 98% of the patients with positive Hutchinson's sign as opposed to 68% of the patients with negative Hutchinson's sign. Moreover, corneal involvement was observed in 56% of patients with positive Hutchinson's sign, in contrast to 28% in negative cases. Most of the complications occurred within one to four days after skin eruptions. However, it is worthy of note that 4 cases developed corneal ulcers 13 days, 2 months, 2months and 4 months, respectively, after appearance of skin rash. We conclude that all patients with HZV□, especially those with positive Hutchinson's sign, should be evaluated for the orbital complications. Finally, we list the cutaneous distribution of ophthalmic and maxillary divisions of the trigeminal nerve to understand the anatomic basis of Hutchinson's sign. |
