中文摘要 | 目的:本研究是探討100例多胞胎妊娠經減胎為雙胞胎之後的懷孕過程和結果。 方法:從1989年3月至1998年5月,共有100位多胞胎的孕婦接受減胎術。所有手術 皆經腹部進行。除了4例懷孕從雙胞胎減為單胞胎之外,其餘的66例三胞胎,24例四胞胎 和6例五胞胎皆減為雙胞胎(研究組)。兩組對照組分別為34位三胞胎和210位雙胞胎。此 二對照組沒有接受減胎術,都在1992年至1997年間生產。研究的指標為流產率、早產率、 生產時的懷孕週數以及新生兒的體重。 結果:比較接受減胎術的雙胞胎以及未減胎的對照組雙胞胎,兩者的流產率 (3.1%vs.2.4%),生產時的懷孕週數(35.7週vs.37週)和出生體重(2347克vs.2470克)沒有 差異。比較研究組和未減胎的三胞胎對照組,可以發現研究組的生產週數(35.7週)比對照 組(34.1週)延長,而胎兒的平均出生體重(2347克)則比對照組(1807克)顯著提高。 結論:本研究顯示多胞胎的減胎術不會增加胎兒的流產率和早產率。比起未減胎的參胞 胎,減胎後的雙胞胎其平均出生週數較大,出生體重較重。 |
英文摘要 | Objective: This study was undertaken to evaluate the course and outcome of 100 multi-fetal pregnancies in which fetal reduction to twins was done. Methods: From March 1989 through May 1998, 100 pregnant women carrying multiple fetuses underwent pregnancy reduction by a single operator via a transabdominal approach. Except for four pregnancies that were reduced from twins to singletons, there were 66 triplet, 24 quadruplet, and six quintuplet pregnancies reduced to twins (study group). Two control groups included 34 triplet and 210 twin pregnancies which were delivered during the period from 1992 through 1997 and had not undergone fetal reduction. We compared the pregnancy course, gestational age at birth, and infant's birth weight among the study and control groups. Results: Based on twin deliveries in the study group, the mean gestational age at delivery was 35.7 weeks and the mean neonatal birth weight was 2347 grams. The abortion rate (3.1 % vs 2.4%), the gestational age at delivery (35.7 weeks vs 37 weeks) and the infant's mean birth weight (2347 gm vs 2470 gin) were comparable between the study group and the control twin group. The gestational age at delivery (35.7 weeks) in the study group was higher than the 34.1 weeks in the control triplet group. The infant's mean birth weight (2347 gin) was higher than the 1807 gin in the control triplet group. Conclusion: This study showed that multi-fetal pregnancy reduction did not carry an increased risk of abortion and preterm birth. Both the gestational age at delivery and the birth weight of the neonates in the group with fetal reduction were higher than those in the non-reduced triplet pregnancies. |
