中文摘要 | 本研究為描述性的比較性研究,目的在比較加護病房病人家屬與護理人員對感 受家屬需求的重要程度與需求獲得程度之差異。研究對象取自北中南三家醫學中心、五家 區域醫院的成人加護病房的病人家屬和該病房的護理人員。分別以「加護病房病人家屬需 求問卷調查表」及「加護病房護理人員對家屬需求感受問卷」收集資料。48項需求又分為 五大類需求類別,包括「保證」、「陪伴病人」、「訊息獲知」、「舒適」及「支持」。 共得有效問卷分別為家屬82份、護理人員119份。 所收集的資料以SPSS+PC套裝軟體進行量性資料的處理與分析。研究結果得到以下結 論:(一)加護病房病人家屬與護理人員對家屬48項需求感受的重要程度有22項達顯著性 差異,且在五類需求類別上亦有差異(P<0.001,P<0.05)。即家屬覺得重要的需求項目 ,護理人員卻覺得不重要。或護理人員認為對家屬是重要的需求項目,家屬卻認為不重要 ,即護理人員對感受需求重要程度的平均得分(152.5±14.5)低於家屬對感受需求重要 程度的平均得分(156.0±15.9)。(二)兩者在感受需求是否獲得的情形,有30項達顯 著差異。且家屬感受到需求已經獲得的平均得分低於護理人員。表示家屬認為他們需求已 經獲得的程度為0.54(1分為滿分),而護理人員卻覺得家屬的需求獲得程度已達0.7分,由 此亦可看出護理人員高估了家屬對需求真正獲得的情形。 根據研究結果,加護病房病人家屬與護理人員對感受家屬需求的重要程度與需求獲得 的程度的確有差異。建議可將研究結果運用於臨床實務與行政上,使家屬和護理人員的認 知差異減少,使家屬真正的需求可獲滿足,增進兩者之間的溝通。使其共同促進病人疾病 的康復。提升醫療照護的層次。 |
英文摘要 | By the year 2000 hospitals without family-focused programs of care might not be acceptable as contemporary hospitals. The benefits of caring for the families as well as the patients are well supported in the literature. Since critical care nurses are the major health care resource for families, it is important that they understand the needs of families identified in this research. This study used a descriptive exploratory design. The purposes are: (1) to compare the importance and statisfaction of family needs perceived by nurses and family members of patients' in ICU. (2) to discover which of these needs are perceived by families as being unmet. We used the questionnaire of the Critical Care Family Needs Inventory ( CCFNI) to collect data on a convenience sample of 82 family members and 119 nurses from three medical centers and eight locial hospital's adult ICU. The CCFNI categories of family needs included the following five dimentions: 「 Assurance」、「Proximity」、「Information」、「Comfort」、「Support」. The results indicated that differences exited in 26 items of the important needs and 30 items of perceived satisfaction of needs between families and nurses showed statistically significant (P<0.001,P<0.05). The mean score of the perceived importance by family members (156.0±15.9) was higher than that by nurses(152.5±14.5). But the mean score of perceived satisfaction by family members (0.54±0.22) was lower than that by nurses (0.70±0.15). Family members and nurses ranked differently in the five category needs. The 「 Proximity」 needs was identified by family members as the most important and they are perceived as satisfied by nurses. Whereas nurses identified the 「Information」needs as important and also perceived as satisfied. |
