英文摘要 | Leptin is a novel adipocyte-derived hormone that decreases food intake via its cognate receptor (Ob-R) in the hypothalamus. Leptin also acts as a metabolic signal to the neuroendocrine and reproductive systems. Recently we demonstrated non-adipose tissue production of leptin. Northern blot analysis and immunohistochemical study revealed that leptin is produced by human placenta) trophoblasts and amnion cells, and is secreted into both maternal antf'fetal circulation. Plasma leptin levels in severe preeclamptic women were elevated significantly compared to gestational age- and body mass index (BMD-matched normotensive pregnant women. Northern blot analysis revealed that leptin mRNA levels are increased in the placentas from severe preeclamptic women compared to normal pregnant women. These results indicate that leptin production in placenta is augmented in preeclampsia, suggesting its local significance in placenta since placenta expresses leptin receptor, Ob-R. We also found intense immunohistochemical staining for leptin in trophoblast cells of hydatidiform mole, and augmented leptin gene expression in molar tissues. Plasma leptin levels in women with hydatidiform mole were 2-fold higher than those in age- and BMI-matched normal pregnant women in the first trimester. Plasma leptin level decreased to that of normal nonpregnant women within a few days of removal of molar tissue. These findings indicate that the elevated plasma leptin in patients with hydatidiform mole originates mainly from molar tissue, thereby suggesting that hydatidiform mole is a leptin-producing tumor. This paper reviews the recent studies on leptin production in human placenta and discuss the physiologic and pathophysiologic significance of leptin in normal and abnormal pregnancy. |
