英文摘要 | Since 1993, the author attempted a fractionated afterloading brachytherapy on 7 patients, aged from 23 to 79 years, with recurrent malignant astrocytomas. Three tumors invaded the basal ganglia. Patients were treated with intracatheter temporary implant of high-does-rate Iridium. Three elder patients directly underwent the stereotactic procedure due to poor physical condition and four patients were irradiated firstly after massive debulking of main tumors. Each patient received one fraction, about 3-5 minutes every two days, for a total of three fractions. A mean delivered does of 3-5 Gy/min was prescribed per fraction at tumor center and it was limited to less than 0.5 Gy/min at 5 cm peripheral to the tumor rim. No surgical mortality happened in this series, but one case mild infection. One patient suffered from late-onset anemia and another two patients required craniotomy to debulk the radionecrotic mass. Overall, their one-year survival rate 86% after primary diagnosis and 51% after radiotherapy, but it decreased to 57% and 34% respectively two-years later. Five patients with glioblastoma multiforme yielded a 6-month and one-year survival of 75% and 38% respectively after brachytherapy. This modality as an alternative for managing malignant gliomas is suggested in recurrent malignant astrocytomas. |
