中文摘要 | 醫療體系在婦女保護工作中扮演重要的角色,是受虐婦女最常使用的社會資源之 一。唯受虐婦女使用醫療經驗如何,醫療體系功能彰顯的程度等均有待研究了解,以建構臺 灣本土之受虐婦女醫療照顧模式。 本研究收集自 1997 年 7 月至 12 月期間, 在南部某綜合醫院家庭醫學科就診之受虐婦女 中,計有 50 位過去曾遭受身體虐待,她們的平均年齡為 32.7 ± 9.8 歲。 在 50 位受虐 個案中有 11 位( 22% )雖曾遭受身體虐待,但未就醫診療。 而有就醫治療 39 人( 78% )中,有 5 人未曾要求開立診斷證明書, 15 人曾被拒開診斷證明書,19 人未被拒開診斷 證明書。比較醫療院所開立受虐婦女診斷證明書方面,顯示醫學中心及公立醫院(含軍系醫 院)有較高比率拒開診斷證明書。就醫診療的場所,在 39 位有就醫經驗的受虐婦女中,有 26 位在門診就醫, 6 位在急診就醫,同時在急診、門診訧醫者有 7 位。在就醫過程中僅 4 人被告知婦女保護的相關資源。 受虐婦女就醫診療的滿意程度為 -0.41, 95% 信賴區間為 -0.69 至 -0.12 。 結論:本研究中,在醫療資源未匱乏的情況下,仍有二成受虐婦女未能使用醫療資源。曾經 使用醫療資源的受虐婦女,有四成曾被拒開診斷證明書,被告知婦保資源者僅一成,而整體 受虐婦女就醫滿意度顯著偏低。結果顯示促進受虐婦女使用醫療資源,以及改善醫療品質以 提升受虐婦女使用醫療資源之滿意度,當為醫療體系參與婦女保護工作所應解決之重要課題 。 |
英文摘要 | Battered women frequently use medical settings as their initial helper in the course of domestic violence. In order to realize their medical care experience, we conducted a survey of battered women in a family medicine clinic of a general hospital in southern Taiwan between July and December in 1997. There were totally 50 battered women who had experienced physical assault in this study, with mean age 32.7 ± 9.8 years. Though being physically assaulted, 11(22%) battered women did not seek any health care provider for help. There were 15 batteed women who were rejected to have a medical certificate, and 19 battered women who were not rejected. Comparing different medical settings, we found that there were significantly higher rejection rates in medical centers and public hospitals (p<0.05). In those 39 battered women treated in the hospital, 26 were treated at the out-patient clinics, 6 at the Emergency unit and 7 at both of them. There were only 4 battered women being ever told about the available resources of women protection by their health care providers. The average score of satisfaction with the medical care was -0.41, with 95% confidence interval -0.69 to -0.12. In conclusion, there were 1/5 of the battered women who had not sought any medical care before, 2/5 who used the medical resources were rejected to have medical certificate and only 1/10 who were ever told about the available resources of women protection. Definitely most of the victims were not satisfied with the medical care they had received. |
