中文摘要 | 執行醫用加速器之接收測試時,通常經由繁鎖的機械測試以確保轉軸之穩定性。本文目的在於使用一個自製裝置來精確地描繪出機械等中心點的軌跡。對於轉臂等中心點,使用複寫紙黏貼於固態假體,假體厚度必須足以穩固地直立於平面上,複寫紙紙面需與轉臂旋轉平面平行。將裝有呈直角狀長針的帽套,套在距離指示器的尾端,並使針尖輕觸紙面。而對於準直儀及治療檯之等中心點,針尖必須指向準直儀旋轉平面。當旋轉過後,軌跡將被刻劃在複寫紙上。本篇中吾人使用Siemens KDS-2及Philips SL-18型兩種直線加速器進行測量。兩種加速器的轉臂等中心點均符合製造商的接收測試規格標準,約在1公釐直徑的圓周之內,準直儀及治療檯之等中心點分別為小於1公釐及1.2公釐直徑的圓周之內。一般而言,對於傳統放射治療其位置再現性約為2至3公釐,則製造商所定訂之等中心點規格是可以接受的。然而,隨著非共平面照野或立體定位放射手術的日趨頻繁使用,為達成高品質的放射治療,明確地定義更嚴格的等中心點的位置是不可或缺的。 |
英文摘要 | The current increasing interest in conformal therapy and stereotactic radiosurgery imposes tighter tolerance than conventional radiotherapy on the operation of medical linear accelerators. At the time of acceptance testing, meticulous mechanical tests are usually conducted to ensure that the axes of rotation are stable. The objective of this study is to provide a self-made device which was designed to trace out the locus of the mechanical isocenter precisely. For a gantry mechanical isocenter, a sheet of carbon paper was taped on a solid phantom which was thick enough to firmly stand erect. The surface of the carbon paper was parallel to the plane of gantry rotation. A needle bent at a right angle was held by a cap at the end of a telescoping column of a front pointer. If then, gently touched the surface of the carbon paper. For the collimator and couch isocenter, the needle was pointed to the plane of collimator rotation and the carbon paper was made parallel to the plane of the collimator rotation. After rotation, the locus was left on the carbon paper. In this study, the machines investigated are the Siemens KDS-2 and the Philips SL-18 linear accelerators(linac). For the gantry isocenter, both types of accelerators match the standards of the manufacturer's acceptance tests in which is a circle within about 1mm diameter of point, The diameters of the point circle which were recorded from the isocenters of the collimator and the couch were less than 1mm and 1.2mm respectively. In general, the manufacturer's specifications of mechanical isocenter are acceptable for conventional radiotherapy in which position reproducibility is on the order of a 2mm to 3mm diameter. However, with the increasing use of noncoplaner field or stereotactic radiosurgery, a well-defined mechanical isocenter is indispensable in performing high quality radiotherapy. |
