中文摘要 | 近年來由於電腦和機器設備的發展進步,使得放射治療技術在電腦斷層攝影(CT)、多葉式準直儀(multileaf collimator)三度空間放射治療計畫(three-dimensional Radiation treatment planning, 3DRTP)系統等設備的幫助下,發展出三度空間順形放射治療(three-dimensional conformal radiation therapy, 3DCRT)。而強度調控放射治療(intensity modulated radiation therapy, IMRT)除了擁有三度空間順形放射治療「順形治療」的功能外,它還具有調控每一入射射束通量「順行調控射束強度」的能力,因此可以準確的且均勻的給予腫瘤細胞適當的劑量以控制腫瘤細胞,同時又能將周圍敏感器官(例如:眼球,脊髓...)之劑量有效降低,如此即可將局部控制(local control)做的更好,提高腫瘤控制率(tumor control probability, TCP)和降低正常組織併發症的機率(normal tissue complication probability, NTCP),進而提高病人存活率,促進整體醫療品質。強度調控放射治療(IMRT)已成為未來放射治療的趨勢及可行的一個方向,其發展將是未來癌病病人的一大福音。本文內容即針對強度調控放射治療技術的發展背景及其基礎理論、應用做說明。 |
英文摘要 | The advances in computer software and hardware over the last decade have led to the development of 3DCRT (three-dimensional conformal radiation therapy) accomplished by CT (computed tomography) MLC (multileaf collimator) and 3DRTP (three-dimensional radiation treatment planning) systems. The new trend in radiation therapy: intensity modulated radiation therapy (IMRT) which has the ability to produce dose distribution which is conformal to targets of irregular shape just as 3DCRT. Even more, with IMRT the beam intensity is made to be proportional to the target thickness as assessed from a beam's eye view as the beam is angled around the patient. So, the dose to the tumor is scalated, while at the same time, minimizing the dose to the surrounding normal tissue (ex: eyes bone marrow?.). There is evidence that IMRT can improve tumor control probability (TCP), and decrease normal tissue complication probability (NTCP). And then patients' surviving probability can improve, the qualities of radiation therapy can promote, too. Here we will describe the theory and practice of intensity modulated radiation therapy. |
