中文摘要 | 為了使珊瑚球(Hydroxyapatite) 植入眼窩的過程較順利無阻力,以及方便把眼 外肌縫回,珊瑚球在植入前大都會先以某種物質包覆,目前最常使用的包覆物為捐贈的鞏膜 。捐贈的鞏膜有傳染疾病的危險性且無法隨時取得, 因此, 有人使用自體的顳筋膜( temporalis fascia )、自體的闊筋膜( fascia lata )或腹直肌鞘( abdominal sheath )來取代鞏膜, 但此方法需另外再開一個傷口,甚不方便。 本研究使用 " Vicryl mesh " 包覆珊瑚球再植入眼窩, 自民國 85 年 12 月至民國 86 年 7 月, 共 16 個病例, 追蹤 2-9 個月, 除了 2 個病例分別在第 3 週及第 14 週發生結膜裂開( conjunctival dehiscence )外,其餘病例並無其他併發症。 Vicryl mesh 包覆後的植入物可順利地植入 眼窩,並可輕易地將眼外肌縫回,且無傳染疾病的危險性,除此之外,Vicryl mesh 為人工 合成材質,取得容易且易於吸收,可促進眼窩植入物的血管增生,為捐贈鞏膜在珊瑚球植入 時的良好替代品。 |
英文摘要 | To facilitate placement within the orbit and allow attachment of the extraocular muscles, hydroxyapatite orbital implants must be wrapped in some materials such as donor sclera, autologous temporalis fascia, autologous fascia lata, or lyodura. The most widely used material is donor sclera. Donor sclera has many disadvantages, including transmitting fatal infectious diseases and being not readily available. We have used Vicryl mesh (polyglactin 910) as the wrapping materials in 16 patients from December 1996 to July 1997. There were no complications during the follow-up period of 2-9 months except 2 patients demonstrated conjunctival dehiscence at 3rd and 14th weeks. Motility associated with the Vicryl mesh-wrapped implants was considered excellent in all patients. The Vicryl mesh-wrapped implant goes into the socket readily, allow one to easily attach the extraocular muscles and has no risk of transmittable diseases. The Vicryl mesh may be a good alternative to donor sclera for wrapping hydroxyapatite implant. |
